There were 186 corpses of males aged 22-82 years and 109 corpses of females aged 32-93 years. All of them died from accidental causes not associated with pelvic organ diseases. Dissection, vascular injection and statistical analysis were used.

In males, SGA forms anastomoses with other branches of internal iliac artery in 24.2% of cases on the right and in 21.0% of cases on the left. Similar collaterals in females are observed in 13.8% of cases on the right and in 15.6% of cases on the left. Mean length of intrapelvic anastomoses of a. glutea superior in males is 1.4 cm, mean diameter - 2.1 mm. Mean length of such collaterals is 2.0 cm in females, mean diameter - 2.2 mm. As a rule, anastomoses are observed in proximal and middle thirds of intrapelvic segment of SGA. Distal anastomoses are rare. Linear correlation between the diameters of a. glutea superior and intrapelvic anastomoses was not detected in females but was fixed in males.

To determine an optimal approach in the treatment of patients with intestinal fistulae.

We have used the above-mentioned algorithm in the treatment of 46 patients. The majority of patients (
=44, 96%) were transferred under supervision of our multidisciplinary team with severe water-electrolyte disturbances and signs of secondary protein-energy malnutrition (PEM) that required complex combined nutritional support. A two-stage approach was applied in all cases. The first one was conservative and included correction and prevention of infectious complications, nutritional and metabolic therapy, local wound treatment, and anatomical assessment of the fistula. The second stage was reconstructive and implied various reconstructive interventions not earlier than 3 months after formation of the fistula.

Conservative approach was followed by fistula healing in 6 patients, surgery was required in 25 patients. Complications with subsequent redo surgery occurred in 4 cases. In all cases, favorable outcome was noted. All 25 patients were discharged. Three patients refused reconstructive surgery after development of fistula, they were also discharged. Mortality rate was 26% (

In period from December 2018 till March 2020 78 patients underwent pancreatoduodenectomy for pancreatobilliary tumors in department of abdominal surgery of National Medical Research Center of Surgery named after A.V. Vishnevsky. Intraoperative frozen section investigation of pancreatic functioning acinar structures (FAS) was held for all patients. 38 patients had more than 40% of FAC and were related with high risk of pancreatic fistula (PF), while 40 patients with less than 40% FAC were included in low risk of PF group. In both groups patients were randomized to main and control subgroups.
