Since the beginning of Vladimir Putin's rule in Russia 20 years ago, Russia has lost much of the leverage that it previously enjoyed in the Baltics. On the one hand, Russia has antagonized Baltic states through its wars in Georgia and Ukraine, on the other hand, deep integration in the EU and NATO has pulled the Baltic states from the Russian zone of influence. This article discusses the gradual decrease of Russia's influence in the Estonia, Latvia and Lithuania while considering the increased NATO presence in the region, Baltic energy security, and Baltic Russian-speaking communities.The Vladimir Putin regime has framed the Russian national idea in terms of security and militarization, both domestically and internationally. This unifying vision emerged as a reaction against the 1990s and blended together a cultivation of militant and anti-Western patriotism, Soviet nostalgia, and religious orthodoxy. Emphasis on cultivation of the youth has been another key defining feature of these efforts. This vision required a growing Russian presence regionally and globally, which in turn necessitated restoration of Russian military power. It resulted in growing self-confidence, internal repressions, and external military interventionism. There are glimmers of hope, but the Putin regime has shown that it will fight to stay in power, and the cult of militarization which has permeated multiple aspects of the Russian society will influence Russia's uncertain future.In his 20 years in power, Vladimir Putin has employed a savvy use of social policy to bolster his popularity, but it is a set of policies not well designed to address Russia's chronic under-provision of healthcare, education, and other social services-particularly in rural areas. The COVID-19 global pandemic will only exacerbate Russia's social policy challenges and could pose a threat to Putin's political survival.Self-mixed optical feedback interferometry based laser sensors show promising results in the measurement of the vibration frequency. To date several measurement methods have been developed to extract the vibration information from the self-mixed (SM) signal; however, the complexity and accuracy of the methods still need improvement. The presented work tries to fulfill the gap by realizing a novel method using maximal overlap discrete wavelet transformation (MODWT) and multi-resolution analysis (MRA). The proposed method can reconstruct the micro-harmonic ( less then 5 μ m ) vibration up to 1 kHz even under weak feedback conditions. The mean squared error and the maximum relative error of the proposed method for this range remained below 1.89 × 1 0 - 3 & 8.79%, respectively. Although, above 1 kHz, the proposed method turns out to be futile to reconstruct the vibration signal but still capable to measure vibration frequency up to 10 kHz with an accuracy of ± 0.0001. The method also found suitable to measure non-sinusoidal vibration frequency with reasonable accuracy even for the moderate feedback conditions. The authors envision that the proposed method will provide a compact, non-contact, and low-cost alternative for the vibration frequency measurement hence useful in early fault detection schemes and lung abnormality diagnosis.In this study, it was aimed to investigate the effects of both using curcumin and microencapsulation method on in vitro release behaivour of chia seed oil and its antioxidant potential during simulated gastrointestinal (GI) tract. Maltodextrin (MD) and gum Arabic (GA) was used as wall materials for freeze dried capsules. Orthorhombic RMnO3 (R = rare-earth cation) compounds are type-II multiferroics induced by inversion-symmetry-breaking of spin order. They hold promise for magneto-electric devices. However, no spontaneous room-temperature ferroic property has been observed to date in orthorhombic RMnO3. Here, using 3D straining in nanocomposite films of (SmMnO3)0.5((Bi,Sm)2O3)0.5, we demonstrate room temperature ferroelectricity and ferromagnetism with TC,FM ~ 90 K, matching exactly with theoretical predictions for the induced strain levels. Large in-plane compressive and out-of-plane tensile strains (-3.6% and +4.9%, respectively) were induced by the stiff (Bi,Sm)2O3 nanopillars embedded. The room temperature electric polarization is comparable to other spin-driven ferroelectric RMnO3 films. Also, while bulk SmMnO3 is antiferromagnetic, ferromagnetism was induced in the composite films. The Mn-O bond angles and lengths determined from density functional theory explain the origin of the ferroelectricity, i.e. modification of the exchange coupling. Our structural tuning method gives a route to designing multiferroics.BACKGROUND Benign esophageal tumors are considered rare, and most commonly are leiomyomas with the incidence of 0.005%. They arise intramurally in the distal two-thirds of the esophagus and are multiple in about 5% of patients. Leiomyoma lesions can mimic esophageal cancer making the diagnosis more challenging. Many cases are asymptomatic, up to 15%-50%, and most cases are discovered incidentally during esophagogastroduodenoscopy (EGD) and other procedures. The treatment of choice for symptomatic leiomyomas to relieve the compression is surgical enucleation with either an open thoracotomy, submucosal tunneling endoscopic resection, a video-assisted thoracoscopic approach (VATS), or with robotic techniques. CASE REPORT Here we report a case of a 53-year-old Saudi female patient who presented with dysphagia which had been ongoing for a long time. There were no other associated symptoms such as dyspepsia, nausea, or vomiting. She was diagnosed with a calcified leiomyoma of the esophagus based on a computed tomography (CT) scan and an endoscopic ultrasound (EUS) finding. After thorough investigations and workups, the management plan was taken at King Faisal Specialist Hospital and Research Centre by preforming a right video-assisted thoracoscopic enucleation. CONCLUSIONS The aim of this case report was to add to the literature by reporting the satisfactory outcomes of right video-assisted enucleation contrary to other surgical approaches that have been discussed in the literature.The purpose of this study was to evaluate the effect of placing small lead shields on the temple region of the skull to reduce radiation dose to the lens of the eye during interventional fluoroscopically-guided procedures and cone-beam computed tomography (CBCT) scans of the head. EGSnrc Monte-Carlo code was used to determine the eye lens dose reduction when using lateral lead shields for single x-ray projections, CBCT scans with different protocols, and interventional neuroradiology procedures with the Zubal computational head phantom. A clinical C-Arm system was used to take radiographic projections and CBCT scans of anthropomorphic head phantoms without and with lead patches, and the images were compared to assess the effect of the shields. For single lateral projections, a 0.1 (0.3)-mm-thick lead patch reduced the dose to the left-eye lens by 40% to 60% (55% to 80%) from 45° to 90° RAO and to the right-eye lens by around 30% (55%) from 70° to 90° RAO. For different CBCT protocols, the reduction of lens dose with a 0.3-mm-thick lead patch ranged from 20% to 53% at 110 kVp. For CBCT scans of the anthropomorphic phantom, the lead patch introduced streak artifacts that were mainly in the orbital regions but were insignificant in the brain region where most neurointerventional activity occurs. Echocardiography (echo)-guided PBPV can completely avoid the use of radiation and contrast agents compared to fluoroscopy-guided PBPV. Although we have confirmed that echo-guided PBPV is feasible in humans, the poor visibility of the traditional catheter under echo greatly limits the promotion of this new technology. Methods We produced a novel double-balloon catheter to make the catheter easy to be detected by echo through adding a guiding balloon at the distal end of the catheter. Echo-guided PBPV was performed on thirty healthy swine using either a novel catheter or a traditional catheter to evaluate the feasibility and safety of the novel double-balloon catheter. The feasibility was evaluated by the success rate of balloon inflation at the pulmonary valve annulus and the operating time. The safety was evaluated by the frequency of balloon slippage and the incidence of complications. Results There were no significant between-group differences in terms of weight and the ratio of balloon diameter to pulmonary annulus diameter. The success rate was 93.3% and 60% in the novel and traditional groups, respectively. The novel group had significantly (p less then 0.05) lower mean procedure time (6.33±6.86min vs 24.8±9.79min) and lower frequency of balloon slippage (0.07±0.26 vs 0.53±0.52), arrhythmia (0.07±0.26 vs 0.47±0.52), and tricuspid regurgitation (6.7% vs 40%) than the traditional group. No myocardial hematoma or pericardial tamponade occurred in the novel catheter group. Objective To evaluate the degree of implementation of reflex testing in 2019 and to know the implementation of rapid diagnostic and/or dried blood spot testing (RDT and / or DBS) in Spanish hospitals. Methods Cross-sectional study through a survey conducted in October 2019 to Spanish general hospitals with at least 200 beds, public or private with teaching accreditation. Results 129 (80%) hospitals responded. Reflex testing is performed by 89% of the centers vs. 31% in 2017 (P less then .001). From 2017 to 2019, centers using alerts to improve continuity of care increased from 69% to 86% (P=.002). In 2019, 11% of centers can determine anti-HCV in dried spot, 15% viremia in dried spot, 0.85% anti-HCV in saliva, and 37% of antibodies and/or viremia with point of care test. 43% of hospitals have at least one diagnostic method with RDT and/or DBS. Conclusion The implementation of reflex testing has increased significantly, reaching 89% of hospitals in 2019. The recommendations of scientific societies could have contributed to the implementation of reflex testing. On the other hand, access to RDT and/or DBS is insufficient and initiatives are needed to improve their implementation.Combinations of multiple exponentially decaying signals are found across many disciplines of science. Decomposition of these multi-exponential signals into their individual components provides insight into the various contributors to the signal. Magnetic resonance images, for instance, can be acquired with multiple gradient or spin echoes to provide voxel by voxel multi-exponential T2* or T2 decays, respectively. With their millions of voxels, these images make the task of decomposition into individual exponentials computationally challenging. Current implementations take several hours, which is prohibitively long in many settings, such as on-scanner calculation for clinical applications. Here, we present a fast approach for the decomposition of multi-exponential signals. The method is applied to multi echo spin echo MRI scans and computes myelin water maps of the whole brain in under 2min, and luminal water maps of the prostate in under 1min.Background During the last few decades, French armed forces have regularly deployed in asymmetric conflicts. Surgical support for casualties of these conflicts occurs in NATO role 2 and 3 medical treatment facilities (MTF); definitive surgical care occurs in France following a strategic medical evacuation. The aim of this study was to describe the combat injury profile of these soldiers who presented with either non-exclusively orthopedic and/or brain injuries. Methods This descriptive study is a retrospective analysis of the surgical management of French casualties performed in role 2 or 3 MTF in Afghanistan, Mali, Niger, Djibouti and the Central African Republic between January 2004 and December 2014. Results One hundred patients were included. Forty had fragment wounds. The most severe lesions were of the head, neck or thorax. The average injury severity score (ISS) was 34.9 (IC 95% 29.8-40). 17 damage control procedures were performed. The purpose of this study was to determine the indications, technique, and outcomes of patients who had undergone an elbow release and hardware removal following bicolumnar plate fixation for an intra-articular fracture of the distal humerus. Methods Patients who had undergone an elbow release and hardware removal by a single surgeon following bicolumnar fixation of an intra-articular fracture of the distal humerus were identified. Patients were contacted to participate in a chart review and to return to clinic for a follow-up visit. Patients who returned for a follow-up visit completed the DASH (Disabilities of the Arm, Shoulder and Hand), the MEPS (Mayo Elbow Performance Score), and the SF-36. Results Forty-two patients were included in the final analysis. The average time from the original injury to the elbow release procedure was 17.
