The in-vitro cytotoxicity and cell adhesion on the prepared materials were examined by an MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) assay.The target of this work is to investigate and assess the utilization of the synthesized in-situ deposition of metal oxide nanoparticles such as nano-nickel oxide (nNiO), nanocopper oxides (nCuO) and nanoiron oxides (nFe3O4) in aminated cellulose (Acell), as a protected and compelling antibacterial channel of contamination from domestic wastewater. Optimum wavelengths of light for growth and maturation of the gametophytes were observed for both blue (400-500 nm, λmax 453 nm) and green (500-600 nm; λmax 525 nm) lights and were sex-independent. Given the emphasis on food and beverage consumption patterns, the relation between coffee consumption and compliance with the Dietary Guidelines of Americans was also examined. Coffee was consumed by 59% of the sample (n = 8551). Survey-adjusted mean intake among consumers was 544.7 g/day. Percent consumers and mean amounts consumed were highest among adults aged 51-70 years (p less then 0.001), higher income groups (p less then 0.001), and non-Hispanic Whites (p less then 0.001). About 74% of coffee consumers obtained their coffee from stores, 9.8% from fast food restaurants, 4.3% from convenience stores, and 4.2% from someone else. Although in silico analysis revealed that the snail promoter's regulatory fragment contains one GCCG and two SP1 motifs that could be occupied by MRTFs, the genetic study confirmed that MRTF binds only to SP1 sites to promote snail expression. The more accurate studies revealed that MRTF-A binds to both SP1 elements, whereas MRTF-B to only one (SP1near). Although we found that each MRTF alone is capable of inducing snail expression, the direct cooperation of these proteins is required to reinforce snail expression and promote the late stages of EndMT within 48 hours. Furthermore, genetic and biochemical analysis revealed that MRTF-B alone could induce the late stage of EndMT. However, it requires a prolonged time. This systematic review aims to synthesise current global evidence on the effectiveness of HIV prevention interventions for reducing risky sexual behaviour among youth in the last decade.

MEDLINE/PubMed, EMBASE, PsychINFO, ProQuest Central, CINAHL and Web of Science databases, ClinicalTrials.gov and the WHO International Clinical Trials Registry Platform and reference lists of included studies and systematic reviews on effectiveness of HIV prevention interventions for reducing risky sexual behaviour among youth will be searched for articles published from August 2011 to August 2021. Eligible studies will be longitudinal studies including randomised controlled trials and quasi-experimental studies that examined the effectiveness of HIV prevention interventions among youth populatilthcare practitioners and policymakers.

Emotional intelligence (EI) is a concept describing an individual's ability to understand, process and act accordingly on others' and one's own emotions. It is a desirable quality for people working in teams and is beneficial to the individual in many ways. It is increasingly recognised that understanding and developing EI are important to working in the healthcare environment. It becomes especially pertinent in the deployed Defence Medical Services (DMS), where clinicians often fulfil a significant leadership role in a small team.

This study was performed with the aim of exploring the role of EI in military doctors in the UK DMS. EI was measured in a cohort of 64 military doctors by way of a validated self-assessment questionnaire. Results were tested for differences in EI between different services and career stages.

The survey had an 83% return rate. There was roughly equal distribution between service, career groups and ages across the study population. There were more men than women. The populationelection processes also pertain to high EI without it being specifically measured for. The dangers of having too high EI and the benefits of having a range of EI within a team were discussed. There were limitations of being unable to obtain qualitative data and not including primary healthcare staff. Several recommendations were set out as to how the DMS may use or further investigate EI with regard to training and recruitment.
Providing safe and effective anaesthesia in the context of humanitarian medicine is always a challenging situation. Spinal anaesthesia, and in particular its thoracic approach, represents a promising technique for such a limited-resource environment. This prospective observational study investigated the feasibility of thoracic spinal anaesthesia (TSA) for abdominal surgery in a field-deployed military hospital.

We included adults scheduled for elective open cholecystectomy in a field hospital. Patients received TSA at the T9-T10 level. The primary outcomes were the feasibility of surgery under TSA and the haemodynamic/respiratory stability of this anaesthetic technique. The secondary outcomes included patient satisfaction and surgeon comfort regarding the anaesthesia technique and postoperative events (nausea and vomiting, urinary retention, postdural puncture headache).

Surgery was performed successfully in 61 patients under TSA (90% female, 53±13 years old). Intraoperative pain scores were low, with a median Numeric Rating Scale score of 0 (IQR 0-2). Surgeon and patient satisfaction scores were excellent. The haemodynamic and respiratory parameters remained stable throughout the surgery. The incidence of postoperative events was low (nausea/vomiting=8%). None of our patients presented with postdural puncture headache or urinary retention.

In this review article, we will define infographics and visual abstracts, describe why they are useful, outline how to create them, and explain how researchers, educators, and clinicians can use them effectively. We share resources and a stepwise approach that allows readers to create their own infographics and visual abstracts for research dissemination, medical education, and patient communication.Clinicians, researchers, and the public frequently turn to digital channels and social media for up-to-the-minute information on novel therapeutics and vaccines. The value of credible infectious diseases drug information is more apparent in the setting of the coronavirus disease 2019 (COVID-19) pandemic. This viewpoint by the Society of Infectious Diseases Pharmacists (SIDP) provides guidance on utilizing social media platforms to optimize infectious diseases pharmacotherapy. It includes tips for all levels of users but primarily serves a guide for the infectious diseases clinician who has not yet joined social media. Journal clubs have been an enduring mainstay of medical education, and hosting these on social media platforms can expand accessibility and engagement. We describe the creation and impact of #IDJClub, an infectious diseases (ID) Twitter journal club.

We launched #IDJClub in October 2019. Using the account @IDJClub, an ID physician leads a 1-hour open-access Twitter discussion of a recent publication. All participants use the hashtag #IDJClub. Sessions started monthly, but increased due to demand during the coronavirus disease 2019 (COVID-19) pandemic. We used Symplur 's Healthcare Hashtag project to track engagement of #IDJClub per 60-minute discussion plus the following 30 minutes to capture ongoing conversations. We also conducted an online anonymous survey using Likert scales and open-ended questions to assess educational impact.

In its first 20 months, 31 journal clubs were held, with medians of 42 (interquartile range [IQR], 28.5-60) participants and 312 (IQR, 205-427.5) tweets per session. 134 participants completed the survey, of whom 39% were ID physicians, 19% pharmacists, 13% ID fellows, and 10% medical residents. Most agreed or strongly agreed that #IDJClub provided clinically useful knowledge (95%), increased personal confidence in independent literature appraisal (72%), and was more educational than traditional journal clubs (72%). The format addressed several barriers to traditional journal club participation such as lack of access, subject experts, and time.

This can serve as a barrier to professional advancement at all stages of career development. Identifying and connecting with these potential mentors/sponsors outside of one's institutional space can be challenging. For this reason, leveraging social media to develop these professional relationships through flattened hierarchies can allow for professional networking beyond traditional mechanisms. Moderate to vigorous physical activity (MVPA) outside of exercise training significantly correlated with leg press strength (r = 0.700, p = 0.008) in IT, and cardiorespiratory fitness (r = 0.440, p = 0.013) when groups were combined. Concentrations of NGF did not differ between groups, and in IT, BDNF was positively related to LPA outside of training and was significantly lower (87 ± 28.5 pg/mL) than in OT (137 ± 54 pg/mL; p=0.010). Limited research, however, has examined the efficacy of BWT on cardiorespiratory fitness. This is especially true for simple BWT protocols that do not require extraordinarily high levels of effort. We examined the effect of a BWT protocol, modelled after the original "Five Basic Exercises" (5BX) plan, on peak oxygen uptake (VO2peak) in healthy, inactive adults (20 ± 1 y; body mass index 20 ± 5 kg/m2; mean ± SD). Participants were randomized to a training group that performed 18 sessions over six weeks (n=9), or a non-training control group (n = 10). Two age ranges (18 - 35yr, young; > 50yr, retired) were divided into weight bearing athlete and control groups; young athlete (YA), young control (YC), retired athlete (RA) and retired control (RC). Static balance was quantified using force platform derived sway velocity (mm.s-1) and C90area (mm2) data (stable and unstable surfaces, eyes open and closed) Dynamic balance was assessed using the Y balance test (YBT). Results demonstrated significant age effect across groups. However, an athletic effect was evident only assessing dynamic balance and static time to error variables. Mean time to error data (YA, 27.8 ± 5.8; YC, 20.5 ± 11.1; RA, 9.4 ± 8.5; RC, 8.6 ± 9.1 s) recorded significant age and athletic effects for the most challenging condition completed (single leg stance, eyes closed, stable surface). Fourteen untrained subjects (23.9 ± 3.6 years old) underwent RE sessions (leg press and hack squat three sets of 8-12 reps at 70% 1RM) supplemented with leucine (LEU two daily doses of 3g) or a placebo (PLA), separated by a seven-day washout period. Following each occasion, participants were evaluated in three subsequent days (24h, 48h, and 72h) for muscle recovery via a repetition-to-failure test. The following markers were assessed repetition performance, perceived exertion, lactate, creatine kinase, muscle soreness (DOMS), testosterone, and cortisol. No significant difference was observed between LEU and PLA conditions (p > 0.05). Number of repetitions performed in the repetition-to-failure tests, perceived exertion, cortisol, and testosteronecortisol ratio did not change over time (p > 0.05). Creatine kinase increased immediately after exercise, at 24h, and 48h, and was attenuated at 72h post-exercise, while testosterone, lactate, and DOMS increased at 24h post-exercise (p less then 0.05) and remained elevated up to 72h. This research determined if caffeine improved exercise tolerance during repeated-sprint exercise. iCV is a running velocity that distinguishes intermittent running velocities (velocities ≤ iCV) that are sustainable from those resulting in a predictable time to exhaustion (velocities > iCV). Seven physically active men (age = 21.6 ± 1.5 years, body mass = 72.8 ± 5.1 kg, VO2max = 56.9 ± 9.8 mL/kg/min) ingested caffeine (5 mg/kg) or placebo (crossover design) 60 min prior to an intermittent critical velocity (iCV) test. The treadmill grade and velocity at VO2max (vVO2max) were used for iCV testing, and consisted of 3 bouts (10 sec running and 10 sec passive rest) at 130, 110 and 120% vVO2max. Each bout continued until volitional exhaustion and was separated by 20 min of passive rest. Total distance and duration were recorded to determine exercise tolerance using the iCV model. Caffeine ingestion increased running duration at 110% vVO2max (p = 0.02), but not at 120 (p = 0.93) and 130% vVO2max (p = 0.14). Caffeine did not improve iCV model parameters. A single dose of caffeine consumed 60 min before repeated-sprints can improve performance at 110% vVO2max, but not at higher velocities.The purpose of this study was to verify whether cooling between sets during high-intensity resistance exercise improves physical performance and to compare performance among different sites of cooling. It is important because delaying the muscular fatigue could improve total volume at a training session which could lead to greater hypertrophy. Nine healthy and recreational resistance training experienced men, performed six tests of a biceps curl exercise on different days. The first test was the one-repetition maximum test (1RM). Following, we applied five sessions, in crossover and randomized (counter-balanced) design. link2 The subjects received different cooling strategies in each session for 1-min (inter-set rest interval) Control (C) (no Cooling); Palm Cooling (PC); Neck Cooling (NC); Local Cooling (LC) or Tunnel Temperature Cooling (TTC). We analyzed the maximum number of repetitions and the rating of perceived exertion (RPE). The Bayesian analysis showed that no cooling strategy was able to improve performance compared to control, and just NC, when compared to control, showed a 71% probability of increasing the total volume of repetitions. Also, RPE was not modulated by any cooling strategy compared to control, but NC has a chance to reduce individuals' RPE by 52%. In conclusion, no cooling strategy was efficient to improve physical performance during a high-intensity resistance exercise.Several recent reports have indicated positive health and exercise benefits of (-)-epicatechin-rich cocoa products. This study investigated the influence of dark chocolate (DC) supplementation on resting and steady state exercise metabolism in a group of athletically fit females. Using a randomized, single-blind design, 18 exercise trained female subjects were assigned to a 30-d supplementation with either 20g · d-1 of 70% DC (n = 9) or a calorically matched white chocolate (WC) (n = 9). Pre-supplementation (PRE), subjects underwent indirect calorimetry assessment for resting energy expenditure (REE) and exercise energy expenditure (EEE) consisting of steady state cycling for 20 min, 10 min each at 50 W (EEE-50) and 100 W (EEE-100). Upon completion of the 30-d supplementation (POST), subjects repeated the assessment for REE, EEE-50, and EEE-100. Post supplementation REE was significantly increased by ~9.6% in the DC group (Δ REE DC 140 ± 132, WC -3 ± 92 kcal · d-1, p = .017). Post supplementation, neither EEE-50 (DC 4.51 ± 0.59, WC 4.51 ± 0.32 kcal · min-1) nor EEE-100 (DC 6.56 ± 0.60, WC 6.69 ± 0.42 kcal · min-1) were significantly different between groups (p ≥ .05). There were no significant within or between group time effects for substrate utilization at rest or during EEE-50 or EEE-100 (p ≥ .05). To our knowledge this is the first study to demonstrate that a relatively small daily dosage of DC can significantly elevate REE. However, it does not impact steady state EEE or substrate utilization in a group of athletically fit females.The purpose of this study was to investigate the influence of acetic acid (apple cider vinegar; ACV) supplementation on resting and exercise energy expenditure and substrate utilization. Using a randomized, double blind, crossover design, 16 healthy subjects were supplemented for 4 d with either ACV (30-ml/d) mixed in 1 L of a non-nutritive lemon-flavored drink or a placebo (PLA). They were then assessed via indirect calorimetry for resting energy expenditure (REE) and substrate utilization. This was immediately followed by the assessment of steady state cycling exercise energy expenditure at 40 W (EEE-40) and 80 W (EEE-80) and substrate utilization. link3 Results Neither REE nor resting substrate utilization were significantly different between groups (p ≥ .05). During cycling exercise at both 40W and 80W, there were no significant differences observed between groups for energy expenditure (EEE-40 ACV 4.13 ± 0.79, PLA 4.37 ± 0.61 kcal/min; EEE-80 ACV 6.09 ± 0.87, PLA 6.26 ± 0.72 kcal/min) or substrate utilization (40W carbohydrate ACV 0.72 ± 0.19, PLA 0.76 ± 0.16; fat ACV 0.15 ± 0.07, PLA 0.16 ± 0.06 g/min), (80W carbohydrate ACV 1.28 ± 0.32, PLA 1.34 ± 0.35; fat ACV 0.14 ± 0.10, PLA 0.14 ± 0.10 g/min) (p ≥ .05). Conclusions Recent findings suggest that chronic acetic acid supplementation is associated with significant reductions in body weight and body fat; however, the findings of the present study suggest that a semi-acute (4 d) acetic acid supplementation does not impact resting or exercise energy expenditure or substrate utilization.

The in-vitro cytotoxicity and cell adhesion on the prepared materials were examined by an MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) assay.The target of this work is to investigate and assess the utilization of the synthesized in-situ deposition of metal oxide nanoparticles such as nano-nickel oxide (nNiO), nanocopper oxides (nCuO) and nanoiron oxides (nFe3O4) in aminated cellulose (Acell), as a protected and compelling antibacterial channel of contamination from domestic wastewater. The prepared Acell and nNiO/Acell, nCuO/Acell and nFe3O4/Acell nanocomposites were characterized by field emission-scanning electron microscopy (FE-SEM), Fourier transform-infrared spectroscopy (FT-IR), energy-dispersive X-ray spectroscopy, transmission electron microscopy (TEM), selected area diffraction pattern (SAED) and X-ray diffraction techniques (XRD). TEM declared the synthesis of nNiO, nCuO and nFe3O4 with regular size of 10, 23 and 43 nm, correspondingly. The antibacterial impact of both nNiO/Acell, nCuO/Acell and nFe3O4/Acell nanocomposites was inspected against Gram-positive microorganisms (Enterococcus faecalis, Staphylococcus aureus) and Gram-negative microbes (Escherichia coli, Salmonella typhi) utilizing agar disk diffusion routes. Furthermore, the ability of the synthesized nanocomposites as sterilizers for optional domestic wastewater was studied. The sex-dependent effects of different environmental factors on the growth and maturation characteristics of the gametophyte stage were investigated using response surface methodology. Optimum temperature and light intensity were higher for males (20.7-20.9 °C and 28.6-33.7 µmol m-2 s-1, respectively) than females (16.5-19.8 °C and 26.9-32.5 µmol m-2 s-1), and maturity progressed more quickly in males than females. Optimum wavelengths of light for growth and maturation of the gametophytes were observed for both blue (400-500 nm, λmax 453 nm) and green (500-600 nm; λmax 525 nm) lights and were sex-independent. These characteristics were consistent among the three regional lines. Slower growth optima and progress of maturation could be important for female gametophytes to restrict fertilization and sporophyte germination to the lower water temperatures of autumn and winter, and suggest that the female gametophyte may be more sensitive to temperature than the male. The sexual differences in sensitivity to environmental factors improved the synchronicity of sporeling production.A growing amount of evidence prompts us to update the first version of recommendations for lung ultrasound in internal medicine (POLLUS-IM) that was published in 2018. The recommendations were established in several stages, consisting of literature review, assessment of literature data quality (with the application of QUADAS, QUADAS-2 and GRADE criteria) and expert evaluation carried out consistently with the modified Delphi method (three rounds of on-line discussions, followed by a secret ballot by the panel of experts after each completed discussion). Publications to be analyzed were selected from the following databases Pubmed, Medline, OVID, and Embase. New reports published as of October 2019 were added to the existing POLLUS-IM database used for the original publication of 2018. Altogether, 528 publications were systematically reviewed, including 253 new reports published between September 2017 and October 2019. In this context, we consider macro-diversity achieved by the multiple UAV BSs, which are randomly distributed in a spherical swarm. Using the widely used channel model incorporated with the distance-based random blockage effects, which is proposed based on stochastic geometry and random shape theory, we investigate the outage performance of the mmWave UAV swarm network. The present analyses of one and two 24-h dietary recalls for 14,865 persons aged ≥20 years in the 2011-2016 National Health and Nutrition Examination Survey (NHANES 2011-2016) aimed to identify socio-demographic predictors of coffee consumption and to examine whether coffee purchase locations differed by population sub-group. Given the emphasis on food and beverage consumption patterns, the relation between coffee consumption and compliance with the Dietary Guidelines of Americans was also examined. Coffee was consumed by 59% of the sample (n = 8551). Survey-adjusted mean intake among consumers was 544.7 g/day. Percent consumers and mean amounts consumed were highest among adults aged 51-70 years (p less then 0.001), higher income groups (p less then 0.001), and non-Hispanic Whites (p less then 0.001). About 74% of coffee consumers obtained their coffee from stores, 9.8% from fast food restaurants, 4.3% from convenience stores, and 4.2% from someone else. Coffee source locations also varied by age, education, income, and race/ethnicity. Coffee consumers had significantly higher Healthy Eating Index (HEI-2015) and higher Nutrient-Rich Foods (NRF9.3) scores in energy-adjusted models and significantly higher HEI 2015 scores in multivariable models. In multivariable models, coffee consumers had diets with less added sugar (p less then 0.001) but slightly more fat (of all types, including monounsaturated (MUFA), polyunsaturated (PUFA), saturated and solid fats), cholesterol, and alcohol. link3 Their diets had more potassium and magnesium (p less then 0.001) but less vitamin C (p less then 0.001). Coffee consumption patterns in the US vary across socio-demographic groups.Endothelial-mesenchymal transition (EndMT) is a crucial phenomenon in regulating the development of diseases, including cancer metastasis and fibrotic disorders. The primary regulators of disease development are zinc-finger transcription factors belonging to the Snail family. In this study, we characterized the myocardin-related transcription factor (MRTF)-dependent mechanisms of a human snail promoter regulation in TGF-β-stimulated human endothelial cells. Although in silico analysis revealed that the snail promoter's regulatory fragment contains one GCCG and two SP1 motifs that could be occupied by MRTFs, the genetic study confirmed that MRTF binds only to SP1 sites to promote snail expression. The more accurate studies revealed that MRTF-A binds to both SP1 elements, whereas MRTF-B to only one (SP1near). Although we found that each MRTF alone is capable of inducing snail expression, the direct cooperation of these proteins is required to reinforce snail expression and promote the late stages of EndMT within 48 hours. Furthermore, genetic and biochemical analysis revealed that MRTF-B alone could induce the late stage of EndMT. However, it requires a prolonged time. Therefore, we concluded that MRTFs might cause EndMT in a fast- and slow-dependent manner. Based on MRTF-dependent Snail upregulation, we recognized that TGF-β1, as an MRTF-B regulator, is involved in slow EndMT induction, whereas TGF-β2, which altered both MRTF-A and MRTF-B expression, promotes a fast EndMT process.Lysophosphatidic acid (LPA) signaling is known to play key roles in the initiation and maintenance of various chronic pain models. Here we examined whether LPA signaling is also involved in diabetes-induced abnormal pain behaviors. The high-fat diet (HFD) showing elevation of blood glucose levels and body weight caused thermal, mechanical hyperalgesia, hypersensitivity to 2000 or 250 Hz electrical-stimulation and hyposensitivity to 5 Hz stimulation to the paw in wild-type (WT) mice. These HFD-induced abnormal pain behaviors and body weight increase, but not elevated glucose levels were abolished in LPA1-/- and LPA3-/- mice. Repeated daily intrathecal (i.t.) treatments with LPA1/3 antagonist AM966 reversed these abnormal pain behaviors. Similar abnormal pain behaviors and their blockade by daily AM966 (i.t.) or twice daily Ki16425, another LPA1/3 antagonist was also observed in db/db mice which show high glucose levels and body weight. Furthermore, streptozotocin-induced similar abnormal pain behaviors, but not elevated glucose levels or body weight loss were abolished in LPA1-/- and LPA3-/- mice.

Human immunodeficiency virus (HIV) prevention interventions focused at reducing risky sexual behaviours are an important strategy for preventing HIV infection among youth (15-24 years) who continue to be vulnerable to the disease. This systematic review aims to synthesise current global evidence on the effectiveness of HIV prevention interventions for reducing risky sexual behaviour among youth in the last decade.

MEDLINE/PubMed, EMBASE, PsychINFO, ProQuest Central, CINAHL and Web of Science databases, ClinicalTrials.gov and the WHO International Clinical Trials Registry Platform and reference lists of included studies and systematic reviews on effectiveness of HIV prevention interventions for reducing risky sexual behaviour among youth will be searched for articles published from August 2011 to August 2021. Eligible studies will be longitudinal studies including randomised controlled trials and quasi-experimental studies that examined the effectiveness of HIV prevention interventions among youth populatilthcare practitioners and policymakers.

Emotional intelligence (EI) is a concept describing an individual's ability to understand, process and act accordingly on others' and one's own emotions. It is a desirable quality for people working in teams and is beneficial to the individual in many ways. It is increasingly recognised that understanding and developing EI are important to working in the healthcare environment. It becomes especially pertinent in the deployed Defence Medical Services (DMS), where clinicians often fulfil a significant leadership role in a small team.

This study was performed with the aim of exploring the role of EI in military doctors in the UK DMS. EI was measured in a cohort of 64 military doctors by way of a validated self-assessment questionnaire. Results were tested for differences in EI between different services and career stages.

The survey had an 83% return rate. There was roughly equal distribution between service, career groups and ages across the study population. There were more men than women. The populationelection processes also pertain to high EI without it being specifically measured for. The dangers of having too high EI and the benefits of having a range of EI within a team were discussed. There were limitations of being unable to obtain qualitative data and not including primary healthcare staff. Several recommendations were set out as to how the DMS may use or further investigate EI with regard to training and recruitment.
Providing safe and effective anaesthesia in the context of humanitarian medicine is always a challenging situation. Spinal anaesthesia, and in particular its thoracic approach, represents a promising technique for such a limited-resource environment. This prospective observational study investigated the feasibility of thoracic spinal anaesthesia (TSA) for abdominal surgery in a field-deployed military hospital.

We included adults scheduled for elective open cholecystectomy in a field hospital. Patients received TSA at the T9-T10 level. The primary outcomes were the feasibility of surgery under TSA and the haemodynamic/respiratory stability of this anaesthetic technique. The secondary outcomes included patient satisfaction and surgeon comfort regarding the anaesthesia technique and postoperative events (nausea and vomiting, urinary retention, postdural puncture headache).

Surgery was performed successfully in 61 patients under TSA (90% female, 53±13 years old). Intraoperative pain scores were low, with a median Numeric Rating Scale score of 0 (IQR 0-2). Surgeon and patient satisfaction scores were excellent. The haemodynamic and respiratory parameters remained stable throughout the surgery. The incidence of postoperative events was low (nausea/vomiting=8%). None of our patients presented with postdural puncture headache or urinary retention.

This viewpoint by the Society of Infectious Diseases Pharmacists (SIDP) provides guidance on utilizing social media platforms to optimize infectious diseases pharmacotherapy. Finally, we suggest how to effectively interact with and leverage these increasingly important digital platforms.
Journal clubs have been an enduring mainstay of medical education, and hosting these on social media platforms can expand accessibility and engagement. We describe the creation and impact of #IDJClub, an infectious diseases (ID) Twitter journal club.

We launched #IDJClub in October 2019. Using the account @IDJClub, an ID physician leads a 1-hour open-access Twitter discussion of a recent publication. All participants use the hashtag #IDJClub. Sessions started monthly, but increased due to demand during the coronavirus disease 2019 (COVID-19) pandemic. We used Symplur 's Healthcare Hashtag project to track engagement of #IDJClub per 60-minute discussion plus the following 30 minutes to capture ongoing conversations. We also conducted an online anonymous survey using Likert scales and open-ended questions to assess educational impact.

In its first 20 months, 31 journal clubs were held, with medians of 42 (interquartile range [IQR], 28.5-60) participants and 312 (IQR, 205-427.5) tweets per session. 134 participants completed the survey, of whom 39% were ID physicians, 19% pharmacists, 13% ID fellows, and 10% medical residents. Most agreed or strongly agreed that #IDJClub provided clinically useful knowledge (95%), increased personal confidence in independent literature appraisal (72%), and was more educational than traditional journal clubs (72%). The format addressed several barriers to traditional journal club participation such as lack of access, subject experts, and time.

This can serve as a barrier to professional advancement at all stages of career development. Identifying and connecting with these potential mentors/sponsors outside of one's institutional space can be challenging. For this reason, leveraging social media to develop these professional relationships through flattened hierarchies can allow for professional networking beyond traditional mechanisms. Here we aim to describe how individuals can connect through social media to advance their careers and scientific and clinical expertise, advocate for communities, and provide high-quality communication to the public.Social media platforms have revolutionized how we consume information, along with how to effectively present communication, education, and advocacy efforts. Moderate to vigorous physical activity (MVPA) outside of exercise training significantly correlated with leg press strength (r = 0.700, p = 0.008) in IT, and cardiorespiratory fitness (r = 0.440, p = 0.013) when groups were combined. Limited research, however, has examined the efficacy of BWT on cardiorespiratory fitness. This is especially true for simple BWT protocols that do not require extraordinarily high levels of effort. We examined the effect of a BWT protocol, modelled after the original "Five Basic Exercises" (5BX) plan, on peak oxygen uptake (VO2peak) in healthy, inactive adults (20 ± 1 y; body mass index 20 ± 5 kg/m2; mean ± SD). Participants were randomized to a training group that performed 18 sessions over six weeks (n=9), or a non-training control group (n = 10). Two age ranges (18 - 35yr, young; > 50yr, retired) were divided into weight bearing athlete and control groups; young athlete (YA), young control (YC), retired athlete (RA) and retired control (RC). Static balance was quantified using force platform derived sway velocity (mm.s-1) and C90area (mm2) data (stable and unstable surfaces, eyes open and closed) Dynamic balance was assessed using the Y balance test (YBT). Results demonstrated significant age effect across groups. However, an athletic effect was evident only assessing dynamic balance and static time to error variables. Mean time to error data (YA, 27.8 ± 5.8; YC, 20.5 ± 11.1; RA, 9.4 ± 8.5; RC, 8.6 ± 9.1 s) recorded significant age and athletic effects for the most challenging condition completed (single leg stance, eyes closed, stable surface). Fourteen untrained subjects (23.9 ± 3.6 years old) underwent RE sessions (leg press and hack squat three sets of 8-12 reps at 70% 1RM) supplemented with leucine (LEU two daily doses of 3g) or a placebo (PLA), separated by a seven-day washout period. Following each occasion, participants were evaluated in three subsequent days (24h, 48h, and 72h) for muscle recovery via a repetition-to-failure test. The following markers were assessed repetition performance, perceived exertion, lactate, creatine kinase, muscle soreness (DOMS), testosterone, and cortisol. No significant difference was observed between LEU and PLA conditions (p > 0.05). Number of repetitions performed in the repetition-to-failure tests, perceived exertion, cortisol, and testosteronecortisol ratio did not change over time (p > 0.05). Creatine kinase increased immediately after exercise, at 24h, and 48h, and was attenuated at 72h post-exercise, while testosterone, lactate, and DOMS increased at 24h post-exercise (p less then 0.05) and remained elevated up to 72h. Seven physically active men (age = 21.6 ± 1.5 years, body mass = 72.8 ± 5.1 kg, VO2max = 56.9 ± 9.8 mL/kg/min) ingested caffeine (5 mg/kg) or placebo (crossover design) 60 min prior to an intermittent critical velocity (iCV) test. The treadmill grade and velocity at VO2max (vVO2max) were used for iCV testing, and consisted of 3 bouts (10 sec running and 10 sec passive rest) at 130, 110 and 120% vVO2max. Each bout continued until volitional exhaustion and was separated by 20 min of passive rest. Total distance and duration were recorded to determine exercise tolerance using the iCV model. Caffeine ingestion increased running duration at 110% vVO2max (p = 0.02), but not at 120 (p = 0.93) and 130% vVO2max (p = 0.14). Caffeine did not improve iCV model parameters. We analyzed the maximum number of repetitions and the rating of perceived exertion (RPE). The Bayesian analysis showed that no cooling strategy was able to improve performance compared to control, and just NC, when compared to control, showed a 71% probability of increasing the total volume of repetitions. Also, RPE was not modulated by any cooling strategy compared to control, but NC has a chance to reduce individuals' RPE by 52%. This study investigated the influence of dark chocolate (DC) supplementation on resting and steady state exercise metabolism in a group of athletically fit females. Using a randomized, single-blind design, 18 exercise trained female subjects were assigned to a 30-d supplementation with either 20g · d-1 of 70% DC (n = 9) or a calorically matched white chocolate (WC) (n = 9). Pre-supplementation (PRE), subjects underwent indirect calorimetry assessment for resting energy expenditure (REE) and exercise energy expenditure (EEE) consisting of steady state cycling for 20 min, 10 min each at 50 W (EEE-50) and 100 W (EEE-100). Upon completion of the 30-d supplementation (POST), subjects repeated the assessment for REE, EEE-50, and EEE-100. Post supplementation REE was significantly increased by ~9.6% in the DC group (Δ REE DC 140 ± 132, WC -3 ± 92 kcal · d-1, p = .017). Post supplementation, neither EEE-50 (DC 4.51 ± 0.59, WC 4.51 ± 0.32 kcal · min-1) nor EEE-100 (DC 6.56 ± 0.60, WC 6.69 ± 0.42 kcal · min-1) were significantly different between groups (p ≥ .05). There were no significant within or between group time effects for substrate utilization at rest or during EEE-50 or EEE-100 (p ≥ .05). To assess myocardial extracellular volume fraction (ECV) measurement provided by a single-source dual-energy computed tomography (SSDE-CT) acquisition added at the end of a routine CT examination before transcatether aortic valve implantation (TAVI) compared to cardiac magnetic resonance imaging (MRI).

Twenty-one patients (10 men, 11 women; mean age, 86±4.9 years [SD]; age range 71-92 years) with severe aortic stenosis underwent standard pre-TAVI CT with additional cardiac SSDE-CT acquisition 7minutes after intravenous administration of iodinated contrast material and myocardial MRI including pre- and post-contrast T1-maps. Myocardial ECV and standard deviation (σECV) were calculated in the 16-segments model. ECV provided by SSDE-CT was compared to ECV provided by MRI, which served as the reference. Analyses were performed on a per-segment basis and on a per-patient involving the mean value of the 16-segments.

ECV was slightly overestimated by SSDE-CT (29.9±4.6 [SD] %; range 20.9%-48.3%) compared to MRI (29.1±3.9 [SD] %; range 22.0%-50.7%) (P<0.0001) with a bias and limits of agreement of +2.3% (95%CI -16.1%-+20.6%) and +2.5% (95%CI -2.1%-+7.1%) for per-segment and per-patient-analyses, respectively. The purpose of this study was to test the hypothesis that Jaccoud arthropathy (JA) in patients with systemic lupus erythematosus (SLE) is associated with instability of the extensor digitorum (ED) tendons during flexion of the metacarpophalangeal (MCP) joints by comparing the position of the ED tendons between SLE patients with JA and control subjects on hand MRI obtained with flexed and extended MCP joints.

Thirty-two hands of SLE patients with JA (13 women and 3 men; mean age, 50.0±12.2 [SD] years; age range 26-68years) and 24 hands of sex- and age-matched control subjects (20 women and 4 men; mean age, 50.1±13.0 [SD] years; age range 24-68years) were included in the study. This study investigated whether the appropriate antibiotics therapy affects the fecal excretion time in pediatric salmonellosis of different severities and explored the factors associated with the fecal excretion time of nontyphoid Salmonella.

Between 2012 and 2017, admitted children with nontyphoid salmonellosis who consented to receive consecutive stool cultures every 4-7 days until 2 consecutive negative results were obtained were enrolled. Patients were stratified into no, appropriate (bacteremia or severe patients receiving antibiotics active invitro), and inappropriate antibiotics (patients with mild or moderate severity receiving antibiotics or severe receiving antibiotics resistant invitro) therapy groups. A previously proposed severity score was used to classify the patients into severe, moderate, and mild severity classes. The demographics, clinical manifestations, laboratory data and severity were compared among the groups. Twenty-five-hydroxy-vitamin D3 (25-OH-vit D) is a prohormone that is essential for normal calcium homeostasis and bone metabolism. Understanding its role is an important component of the proper care of the pediatric orthopaedic patient. The aim of this study was to determine whether children in Ireland with fractures have increased prevalence of 25-OH-Vit D deficiency compared with age matched controls and to ascertain the relationship between a low 25-OH-vit D level and the incidence of fractures in Irish children. We hypothesised that children presenting to our centre following a fracture would have significantly lower 25-OH-vit D.

A prospective case-control study at a large urban tertiary referral academic hospital located in Dublin, Ireland was completed over a 14 month period from June 2014 to August 2015. Cold therapy is one of the most common nonpharmacologic pain treatments. Despite the existence of many studies about cold therapy, few have examined the effects of cold therapy after abdominal surgery.

The purpose of the study was to investigate the effect of cold therapy applied to the incision area after abdominal surgery on postoperative pain and analgesic use.

This study was a randomized controlled trial METHODS The sample included 60 patients (30 control, 30 experimental) undergoing abdominal surgery. Researchers recorded information from a patient information form, a visual analogue scale (VAS), a pain evaluation form, and the vital signs recording form.

There was no statistically significant difference in pain level between the experimental and control groups as measured by VAS at postoperative hour 1 (p > .05). Furthermore, no statistically significant difference in VAS pain levels between groups was observed at postoperative hours 1, 2, and 8 prior to application of cold therapy (p > .05). Then, when cold therapy was applied at hours 1, 2 and 8, the pain level decreased significantly in the experimental group (p =.001). Pain also decreased in the control group between hours 1 and 8, but this decrease was not as great as that in the experimental group (p=.024).

The aim was to determine musculoskeletal discomfort of nurses and to compare its relationship with their professional quality of life in terms of their socio-demographic characteristics.

The data were collected from 333 nurses working at different units in a public hospital between July and December 2018. The data collection tools include an 8-question participant information form, Cornell Musculoskeletal Discomfort Questionnaire, and Professional Quality of Life Scale.

The nurses suffered mostly from lower back pain (90.4%) and their pain slightly interfered with their works (49.8%). There was a weak positive but significant correlation between musculoskeletal discomfort and professional quality of life of the nurses (r=0.215; p < .001). The nurses, who were younger, were male, were working in executive positions, had less institutional and professional experience, and had higher mean score of musculoskeletal discomfort (p < .05).

The professional quality of life of the nurses experiencing musculoskeletal discomfort in their bodies was negatively affected. For example, as the musculoskeletal disorders increase, the levels of burnout and compassion fatigue, which constitute the quality of life, also increase.

Nearly every patient admitted to a neuroscience intensive care unit (ICU) will experience pain and nurses are tasked with analgesic administration. Within the setting of the ongoing opioid epidemic it is not well understood how nurses meet the need to alleviate pain while individualizing analgesic administration.

This qualitative study used a phenomenological approach to determine nurses' perceptions in pain management of patientswith subarachnoid hemorrhage (SAH).

Prospective qualitative inquiry using phenomenology SETTING The study was conducted in a neuroscience intensive care unit at a university hospital.

Nine neuroscience intensive care unit nurses were enrolled using snowball sampling.

Saturation was reached after nine individual nurse interviews. Hermeneutic cycling analysis was used throughout interviews and codes and themes were developed throughout the interview process. Rigor was established using triangulation, rich and thick descriptions, and member checks.

Emerging themes included discernment and hesitation. Discernment is supported by codes such as "nursing judgement" and "follow the orders." Hesitation is supported by codes such as "clouded exam" and "over sedation." Eight nurses made references to hesitation of administering opioids due to the perception that it would cause a poorer neurological exam. All nurses described a reliance on education, experience, or intuition to guide their decision to administer opioids along with using approved pain scales. Themes were confirmed by member checks, which prompted slight modifications to coding.

Results of this study support that nurses do express apprehension in administering opioids to patients with (SAH). This apprehension leads to hesitation to administer the medication and a thought out discernment process.

BACKGROUND Great progress has recently been made in immunotherapy of solid carcinomas. The advent of immunotherapy with checkpoint inhibitors has brought not only a completely different mechanism of action but also different types of adverse reactions. These adverse reactions are similar to those of autoimmune diseases and are referred to as immune-related adverse events. Most adverse reactions are due to lower grade toxic effects. Neurotoxicity is a less frequent. On the other hand, with the spread of immunotherapy, it can be assumed that adverse events with a lower incidence may occur in a relatively higher frequency due to borader usage of immunotherapeutics and thus, physicians outside of cancer centres may be presented with these events. The main therapeutic intervention is immunosuppressive therapy with corticoids. However, in some cases, or in case of delayed intervention, they may be fatal. PURPOSE Therefore, it is of great importance to increase the physicians and patients knowledge of the possible complications of this promising treatment modality. It is essential to apply proper management and help patients engage positively to prevent these situations occurring. The Editorial Board declares that the manuscript met the ICMJE recommendation for biomedical papers.BACKGROUND Modern immunotherapy based on immune checkpoint inhibitors is an innovative treatment, which is already used in the treatment of a number of malignancies, and many other checkpoint inhibitors have been investigated in clinical trials. Monoclonal antibodies against CTLA-4 (cytotoxic T-lymphocyte antigen-4) and PD-1 (programmed cell death-1) or PD-L1 (programmed cell death-1 ligand) are the most commonly used agents. The side effects of these treatments are similar in nature to those of autoimmune diseases. Recently, increasing evidence has indicated that some adverse effects of immunotherapy are associated with the beneficial effect of this treatment. PURPOSE The aim of this review was to summarize current knowledge of the association between the adverse effects of checkpoint inhibitors and the outcomes of patients treated with this therapy. CONCLUSION The association between the effect of immunotherapy and the occurrence of adverse reactions has been identified in a number of studies. It has been best documented in patients with malignant melanoma, non-small cell lung cancer, and renal cell carcinoma. Many studies published so far are limited by the relatively low number of patients and their retrospective design, leaving many questions still unanswered. The first article bases its conclusion on the assumption that the non-Gaussian genuine band shape previously retrieved for the solvated electron in liquid water is an artifact arising from insufficient electron scattering cross sections used in the retrieval. The second article excludes a multimodal band shape based on a photoelectron spectrum of the solvated electron in water clusters recorded at a single ultraviolet photon energy, and it further assumes that cluster results are transferable to the liquid without further justification. Here, we show that based on current data multimodal distributions cannot be unambiguously excluded. Furthermore, the transferability of cluster results to the liquid can be neither justified nor refuted on the basis of currently available experimental ultraviolet photoelectron spectra.The efficacy of a sunscreen tends to be associated with its sun protection factor (SPF) value, a figure determined in a test that relies on the independence of the SPF value to both UV radiation dose and irradiance. We probe these assumptions when photoinduced product degradation is present, and we estimate that the theoretical limit for their validity is when the sunfilter active molecule relaxation time is faster than ∼10 ns. While such threshold relaxation time should be compatible with the expected ultrafast relaxation mechanisms of sunfilter molecules (picoseconds), recent research on sunfilter photodynamics has identified the existence of much longer-lived molecular states. Such long lifetimes could compromise sunscreen performance and make the SPF value very different in natural sun irradiance conditions than in the solar simulated conditions typically used in SPF determination tests.A novel stratum corneum substitute (SCS) has been developed, and the fundamental mechanism of the dehydration process has been studied using the SCS. After washing with cleansers which contain surfactants, our skin "feels" dehydrated (or hydrated). Although many studies have focused on the effect of surfactants on the regulation of the water loss by the lipid bilayers in the stratum corneum (SC) for a long timescale or at equilibrium, only few studies have focused on the acute effect of the surfactant interaction on dehydration. In addition, the interaction between the surfactant and keratin has been often underappreciated compared to lipid bilayers although keratin is the major nonaqueous component of the SC. Here, we have developed novel SCS models, nonkeratinized (lipid only) and keratinized, to study the effect of keratin hydrolysates on the dehydration rate. We have confirmed that the lipid organizational structure of the SCS was similar to that of the human SC using X-ray scattering. We have revealed that keratin hydrolysates play a significant role in the dehydration rate, accelerating the rate for the short term.
