This study provides new insights into the role of the pancreatic microbiota in the intriguing link between pancreatic cystic lesions and cancer.Mutan is an extracellular sticky polymer having α-1-3 and α-1-6 glycosidic linkages with a large diversity in molecular weights and structures depending on the source. These compounds are reported to be highly thermostable and also have potential physiochemical and biological applications. The main aim of this review is to provide an overview of glucosyltransferases and their role in mutan synthesis. The production strategies and structural properties of bacterial mutans are discussed with a goal to improve production efficiency. The physicochemical features, chemical modifications, potential industrial applications and future prospects are also discussed. Epilepsy is the most common childhood neurological disorder in Nigeria. There is therefore a need to determine the effect of a commonly used anti-seizure medication's on serum folate levels of children. This would provide an evidence-based consideration for folic acid supplementation in children on anti-seizure medication as has been suggested by some studies.

To determine whether serum folate levels were lower in children taking long-term carbamazepine or sodium valproate, compared to a control group.

Serum folic acid levels were measured from well-nourished children between the ages of 1-17 years on carbamazepine and sodium valproate monotherapy and their age/sex-matched controls, using spectrophotometry.

The mean serum folate levels of patients on carbamazepine (43) and sodium valproate (22) were 0.032 mg/l ± 0.009 and 0.028 mg/l  ±  0.008, respectively. The mean folate levels of the controls were 0.046 mg/l  ±  0.03 (
 = 0 001). No statistically significant difference was observed between the serum folate levels of children on the two anti-seizure medications, that is, carbamazepine and valproate.

The children on treatment with carbamazepine and sodium valproate for more than 6 months had statistically significantly lower serum levels of folic acid compared to the standard reference range and controls. There are a growing number of organisations working to address the connections between climate change and health. This article introduces the concept of 'theories of change' - the methodology by which organisations or movements hope to bring about social change - and applies it to the current climate change and health movement in England. Through movement mapping, the article describes and offers reflections on the climate change and health ecosystems in England.

Organisations working on climate change and health in England were identified and publicly available information was collated to map movement characteristics, target stakeholders and methodologies deployed, using an inductive, iterative approach.

HLA-B*1502 as a risk factor of PHT-induced SCARs was reported in numerous studies with inconsistent results. This meta-analysis aimed to establish pooling evidence of this association. Materials & methods Pooled odds ratios (ORs) with 95% CIs were estimated using a random-effects model. Results A total of 11 studies on 1389 patients, were included for the analyses. There was a significant association between HLA-B*1502 and PHT-induced SCAR (pooled OR = 2.29, 95% CI 1.25-4.19, p = 0.008). Furthermore, there was a significant association regarding Stevens-Johnson syndrome/toxic epidermal necrolysis (OR = 3.63, 95% CI 2.15-6.13, p less then 0.001) but no association regarding drug reaction with eosinophilia and systemic symptom. Conclusion The results supported the recommendations of HLA-B*1502 screening before treatment with PHT.
To determine the long-term probability of remission without antiepileptic treatment of common epileptic syndromes and of children without a specific syndromic diagnosis.

All children less than 14 years old with 2 or more unprovoked seizures seen at our hospital between June 1, 1994, and March 1, 2011 (n = 680), were included and prospectively followed up until August 15, 2020. Syndromic diagnosis was made retrospectively but blinded to subsequent evolution, employing the data available at 6 months after diagnosis and under predefined operational criteria.

The Kaplan-Meier estimate of the probability of achieving a remission period of at least 5 years, with neither seizures nor antiepileptic treatment at 14 years was 97% for well-defined childhood epilepsy with centrotemporal spikes, 82% for uncertain childhood epilepsy with centrotemporal spikes, 85% for well-defined Panayiotopoulos syndrome, 88% for uncertain Panayiotopoulos syndrome, 93% for nonfamilial self-limited infantile epilepsy, 100% for familial self-limited infantile epilepsy, 86% for absence epilepsy, 6% for juvenile myoclonic epilepsy, 71% for cryptogenic West syndrome, 72% for patients with no associated neurologic deficits and no specific syndromic diagnosis, 65% for symptomatic West syndrome, and 40% for patients with associated neurologic deficits and no specific syndromic diagnosis.

To investigate the effects of a coffee beverage and two whitening systems on the surface roughness and gloss of glazed Lithium Disilicate Glass-Ceramics (LDGC) for computer-aided design/computer-aided manufacturing (CAD/CAM) systems.

Sixty-eight LDGC disks (12 × 10 × 2 mm) were prepared from blocks of CAD/CAM systems (IPS e.max CAD ceramic). Baseline measurements for surface roughness (Ra) and gloss (GU) were taken using a 3-D optical profilometer and a glossmeter, respectively; then specimens were randomized into four groups (

However, the association between blood pressure (BP) control and thromboembolic events remains under-evaluated in patients with AF. We aimed to identify the relation between BP control and the risk of ischemic stroke and systemic embolism in hypertensive patients with AF. Data on 13,712 consecutive patients with AF (9,505 with and 4,207 without hypertension) were retrospectively analyzed. The hypertensive group was divided into quartiles according to the initial BP, linearly interpolated mean BP, variability independent of the mean of the BP, and time in therapeutic range (<130 mm Hg for systolic BP [SBP] and <80 mm Hg for diastolic BP) during follow-up. The primary outcome was ischemic stroke and systemic embolism. The mean follow-up duration of the study population was median 2.7 years (interquartile range 1.1 to 4.9 years), and the median number of BP measurements was 14 (interquartile range 6 to 25) times. Strictly controlled initial and interpolated mean BP and low variability in controlled BP (variability independent of the mean) were associated with a lower risk of ischemic stroke and systemic embolism for both SBP and diastolic BP. A similar risk was observed in patients with strictly controlled SBP (time in therapeutic range under 130 mm Hg >94%) and those without hypertension. We aim to evaluate gender-based differences in clinical outcomes after TAVI. In the present analysis of the GALILEO (Global study comparing a rivaroxaban-based antithrombotic strategy to an antiplatelet-based strategy after transcatheter aortic valve replacement to optimize clinical outcomes) trial, patients with symptomatic severe aortic stenosis and who underwent successful TAVI were stratified by gender. The primary outcome was major adverse cardiac and cerebrovascular events (MACCEs), a composite of all-cause mortality or thromboembolic events (including any stroke, myocardial infarction, symptomatic valve thrombosis, systemic embolism, deep-vein thrombosis, or pulmonary embolism). Major bleeding was defined as a composite of major, life-threatening, or disabling Valve Academic Research Consortium-2 bleeding. Of 1,644 patients, 813 were female, and 831 were male. At baseline, female patients were older and at higher surgical risk (Society of Thoracic Surgeons risk score 4.7 ± 3.6 versus 3.6 ± 3.0, p <0.0001) than male patients. After adjustment for differences in baseline clinical and procedural parameters, female patients had lower rates of MACCE (hazard ratio [HR] 0.69, 95% confidence interval [CI] 0.49 to 0.96), all-cause mortality (HR 0.54, 95% CI 0.34 to 0.87), and noncardiovascular mortality (HR 0.33, 95% CI 0.15 to 0.75) at a median of 17 months of follow-up. By landmark analyses, these differences appeared to emerge with a longer follow-up time. No significant differences in major, life-threatening, or disabling bleeding, cardiovascular mortality, and stroke were noted. Cylindrical composite resin specimens were subjected to bulk compressive creep test with conventional and modified methodology (with/without introduction of stainless steel hemisphere and preload process). Furthermore, specimens undertook different loading rates ranging from 1N/s to 50N/s. Maximum deformation, creep deformation, permanent set as well as percentage of recovery during the creep and recovery procedure were compared, and surface topography changes before and after preload process was evaluated by laser scanning confocal. There is limited epidemiologic data on sigmoid volvulus (SV) from non-endemic regions. Therefore, we performed a multicenter study to report contemporary outcomes and appraise literature-based methods that pair diagnostic and procedural codes to identify SV.

Using an automated search for patients with 'volvulus' in our system from 2011 to 2021, we reviewed electronic charts to clarify the diagnosis, automatically replicate three strategies to identify SV, and retrieved 6-month outcomes.

30 mentor-medical student pairs from the AWS 2020 virtual mentorship pilot-program were surveyed pre-and post-program to explore 1) feasibility of meetings, and, 2) program's perceived efficacy in fostering professional development skills. Participants responded using a 5-point Likert scale (1=not at all; 5=completely). Proportions of participants in each category were compared.

Proportion of participants perceiving monthly meetings to be completely feasible did not differ from pre-to post-program surveys for mentees (75%(21/28) vs. 71%(12/17); p=0.743) or mentors (71%(17/24) vs 71%(13/18); p=1.00). Compared to pre-program responses, mentees endorsed "completely" (Likert scale 5) improving with regard to their elevator speech (p=0.001), developing their curriculum vitae (p=0.003), ability to network (p=0.021), and acquiring skills for career advancement (p=0.003).

Radioactive iodine (RAI) treatment is considered a rare cause of primary hyperparathyroidism (pHPT).

A multi-institutional retrospective review of patients with pHPT who underwent parathyroidectomy from 1990 to 2020 was completed to evaluate the prevalence and latency time for development of RAI-associated pHPT and determine clinical differences in pHPT patients with or without prior RAI treatment.

The appropriate stent length in frozen elephant trunk replacements (FET) remains debated relative to the risk for paraplegia. However, landing the distal end of the stent beyond the curve of the arch facilitates distal reintervention, which is commonly beyond the 10cm stent coverage when deployed proximal to the left subclavian artery. The aim of this study was to evaluate outcomes following the use of 15cm stent grafts in zone 2 (z2, distal to the left common carotid).

Using our single institution-maintained database, 103 zone 2 FET performed from 2016 to 2020 were reviewed.

Of the 103 z2, a 15cm stent graft was used in 51 operations. The indications for FET included acute and chronic aortic dissection, arch aneurysms, and pseudoaneurysms. The incidence of SCI was 0%. Seven deaths (13.7%) occurred.

The data demonstrates the incidence of post-operative paraplegia to be 0% with 15cm z2 FET. The understanding of SCI in FET should not only include the stent length but also from where it begins.

The fate of phenolic compounds in oil and food during cooking vary according to the type of cooking. From a nutritional point of view, reviews largely suggest a preference for using extra-virgin olive oil at a low temperature for a short time, except for frying and microwaving, for which there appears to be no significant advantages compared to olive oil. However, due to the poorly pertinent use of terminology, the different protocols adopted in studies aimed at the same objective, the different type and quality of oils used in experiments, and the different quality and quantity of PC present in the used oils and in the studied vegetables, the evidence available is mainly contradictory. This review tries to reanalyse the main experimental reports on the fate, accessibility and bioavailability of phenolic compounds in cooking oils and cooked vegetables, by considering different cooking techniques and types of oil and foods, and distinguishing experimental findings obtained using oil alone from those in combination with vegetables. The re-analysis indicates that incomplete and contradictory observations have been published in the last few years and suggests that further research is necessary to clarify the impact of cooking techniques on the phenolic compounds in oil and vegetables during cooking, especially when considering their nutritional properties.Outbreaks and prevalence of infectious diseases worldwide are some of the major contributors to morbidity and morbidity in humans. Pharmacophageous plants are the best source for searching antibacterial compounds with low toxicity to humans. In this study, we identified, for the first time, antibacterial components and action modes of methanol-phase extract from such one edible herbaceous plant Rumex madaio Makino. The bacteriostatic rate of the extract was 75% against 23 species of common pathogenic bacteria. The extract was further purified using the preparative high-performance liquid chromatography (Prep-HPLC) technique, and five separated componential complexes (CC) were obtained. Among these, the CC 1 significantly increased cell surface hydrophobicity and membrane permeability and decreased membrane fluidity, which damaged cell structure integrity of Gram-positive and -negative pathogens tested. A total of 58 different compounds in the extract were identified using ultra-HPLC and mass spectrometry (UHPLC-MS) techniques. Comparative transcriptomic analyses revealed a number of differentially expressed genes and various changed metabolic pathways mediated by the CC1 action, such as down-regulated carbohydrate transport and/or utilization and energy metabolism in four pathogenic strains tested. The analytes were separated on a C18 column by gradient elution. Multiple reaction monitoring mode via electrospray ionization source was used to quantify the analytes in positive mode for TS I, DTS I, TS IIA, CTS, and negative mode for PAL, PCTA, and CFA. The linearity ranges were 0.1-8 ng/mL for TS I and DTS I, 0.2-8 ng/mL for TS IIA, 1-80 ng/mL for CTS, 20-800 ng/mL for PAL and CFA, and 10-4000 ng/mL for PCTA. The developed method was accurate and precise for the compounds. The relative matrix effect was less than 15%, and the analytes were stable for analysis. Adult Wistar rats were subjected to BDL and treated with CS for 7, 14, 21, or 28 days intraperitoneally. We performed histomorphometric analyses on Picrosirius-stained liver sections. Cell death was analyzed according to caspase-3 and cathepsin B activity and using a TUNEL assay. Regeneration was evaluated using PCNA immunohistochemistry. BDL led to increased collagen content with corresponding decreased liver parenchyma. CS treatment reduced total collagen and increased parenchyma content after 21 and 28 days. The treatment also promoted changes in the hepatic collagen type III/I ratio. Based on a Google search and literature mining, a large phytochemical library was generated and employed as ligand molecules. In this investigation, the protein target NS2B/NS3 from Dengue was employed, and around 27 compounds were evaluated in a docking study. Phellodendroside (-63 kcal/mole), quercimeritrin (-59.5 kcal/mole), and quercetin-7-O-rutinoside (-54.1 kcal/mole) were chosen based on their binding free energy in MM-GBSA. The tested compounds generated numerous interactions at Lys74, Asn152, and Gln167 residues in the active regions of NS2B/NS3, which is needed for the protein's inhibition. As a result, the stable mode of docked complexes is defined by various descriptors from molecular dynamics simulations, such as RMSD, SASA, Rg, RMSF, and hydrogen bond. Five different sausage formulations were prepared control sample; samples with 1% and 2% addition of chia seed; samples with 1% and 2% addition of black cumin seed. Sausage samples with the addition of cumin seed were characterized by the highest herbal odor and flavor. The addition of Salvia hispanica and Nigella sativa seed significantly affected the amount of volatile compounds in fermented sausages.

Epilepsy is the most common childhood neurological disorder in Nigeria. Treatment of epilepsy is long-term and sometimes lifelong with anti-seizure medications. https://www.selleckchem.com/products/Rapamycin.html There are conflicting reports on the effect of anti-seizure medications on serum folate. There is therefore a need to determine the effect of a commonly used anti-seizure medication's on serum folate levels of children. This would provide an evidence-based consideration for folic acid supplementation in children on anti-seizure medication as has been suggested by some studies.

To determine whether serum folate levels were lower in children taking long-term carbamazepine or sodium valproate, compared to a control group.

Serum folic acid levels were measured from well-nourished children between the ages of 1-17 years on carbamazepine and sodium valproate monotherapy and their age/sex-matched controls, using spectrophotometry.

The mean serum folate levels of patients on carbamazepine (43) and sodium valproate (22) were 0.032 mg/l ± 0.009 and 0.028 mg/l  ±  0.008, respectively. The mean folate levels of the controls were 0.046 mg/l  ±  0.03 (
 = 0 001). No statistically significant difference was observed between the serum folate levels of children on the two anti-seizure medications, that is, carbamazepine and valproate.

The children on treatment with carbamazepine and sodium valproate for more than 6 months had statistically significantly lower serum levels of folic acid compared to the standard reference range and controls. There are a growing number of organisations working to address the connections between climate change and health. This article introduces the concept of 'theories of change' - the methodology by which organisations or movements hope to bring about social change - and applies it to the current climate change and health movement in England. Through movement mapping, the article describes and offers reflections on the climate change and health ecosystems in England.

Organisations working on climate change and health in England were identified and publicly available information was collated to map movement characteristics, target stakeholders and methodologies deployed, using an inductive, iterative approach.

A total of 98 organisations working on health and climate change (and/or sustainability) were initially identified, of which 70 met the inclusion criteria. Most organisations target two or more stakeholders, with healthcare workers, management structures, and government being most commonly cited. Forty-two thermophilic Bacillus isolates could be identified employing morphological, physiological and the 16S rRNA gene sequencing analyses. The isolates showed a high degree of biological diversity involving 13 Bacillus species and 1 subspecies but were dominated by Bacillus licheniformis. Phylogenetic analysis of B. licheniformis isolates based on the DNA sequencing of gyrA and rpoB genes presented them in two main genetic groups. Most unskilled professionals often present themselves as social workers who do not meet professional standards or perform the best practice.Aim Phenytoin (PHT) is a common anticonvulsant agent known for inducing severe cutaneous adverse reactions (SCARs). HLA-B*1502 as a risk factor of PHT-induced SCARs was reported in numerous studies with inconsistent results. This meta-analysis aimed to establish pooling evidence of this association. Materials & methods Pooled odds ratios (ORs) with 95% CIs were estimated using a random-effects model. Results A total of 11 studies on 1389 patients, were included for the analyses. There was a significant association between HLA-B*1502 and PHT-induced SCAR (pooled OR = 2.29, 95% CI 1.25-4.19, p = 0.008). Furthermore, there was a significant association regarding Stevens-Johnson syndrome/toxic epidermal necrolysis (OR = 3.63, 95% CI 2.15-6.13, p less then 0.001) but no association regarding drug reaction with eosinophilia and systemic symptom. Conclusion The results supported the recommendations of HLA-B*1502 screening before treatment with PHT.
To determine the long-term probability of remission without antiepileptic treatment of common epileptic syndromes and of children without a specific syndromic diagnosis.

All children less than 14 years old with 2 or more unprovoked seizures seen at our hospital between June 1, 1994, and March 1, 2011 (n = 680), were included and prospectively followed up until August 15, 2020. Syndromic diagnosis was made retrospectively but blinded to subsequent evolution, employing the data available at 6 months after diagnosis and under predefined operational criteria.

The Kaplan-Meier estimate of the probability of achieving a remission period of at least 5 years, with neither seizures nor antiepileptic treatment at 14 years was 97% for well-defined childhood epilepsy with centrotemporal spikes, 82% for uncertain childhood epilepsy with centrotemporal spikes, 85% for well-defined Panayiotopoulos syndrome, 88% for uncertain Panayiotopoulos syndrome, 93% for nonfamilial self-limited infantile epilepsy, 100% for familial self-limited infantile epilepsy, 86% for absence epilepsy, 6% for juvenile myoclonic epilepsy, 71% for cryptogenic West syndrome, 72% for patients with no associated neurologic deficits and no specific syndromic diagnosis, 65% for symptomatic West syndrome, and 40% for patients with associated neurologic deficits and no specific syndromic diagnosis.

To investigate the effects of a coffee beverage and two whitening systems on the surface roughness and gloss of glazed Lithium Disilicate Glass-Ceramics (LDGC) for computer-aided design/computer-aided manufacturing (CAD/CAM) systems.

Sixty-eight LDGC disks (12 × 10 × 2 mm) were prepared from blocks of CAD/CAM systems (IPS e.max CAD ceramic). Baseline measurements for surface roughness (Ra) and gloss (GU) were taken using a 3-D optical profilometer and a glossmeter, respectively; then specimens were randomized into four groups (

We aimed to identify the relation between BP control and the risk of ischemic stroke and systemic embolism in hypertensive patients with AF. Data on 13,712 consecutive patients with AF (9,505 with and 4,207 without hypertension) were retrospectively analyzed. The hypertensive group was divided into quartiles according to the initial BP, linearly interpolated mean BP, variability independent of the mean of the BP, and time in therapeutic range (<130 mm Hg for systolic BP [SBP] and <80 mm Hg for diastolic BP) during follow-up. The primary outcome was ischemic stroke and systemic embolism. The mean follow-up duration of the study population was median 2.7 years (interquartile range 1.1 to 4.9 years), and the median number of BP measurements was 14 (interquartile range 6 to 25) times. Strictly controlled initial and interpolated mean BP and low variability in controlled BP (variability independent of the mean) were associated with a lower risk of ischemic stroke and systemic embolism for both SBP and diastolic BP. A similar risk was observed in patients with strictly controlled SBP (time in therapeutic range under 130 mm Hg >94%) and those without hypertension. In conclusion, continuous and strict maintenance of SBP under 130 mm Hg with low variability at outpatient clinic follow-up reduces the risk of ischemic stroke and systemic embolism in patients with hypertension and AF.Gender-based differences in outcomes after successful transcatheter aortic valve implantation (TAVI) in patients without an indication for oral anticoagulation have not been well studied. We aim to evaluate gender-based differences in clinical outcomes after TAVI. In the present analysis of the GALILEO (Global study comparing a rivaroxaban-based antithrombotic strategy to an antiplatelet-based strategy after transcatheter aortic valve replacement to optimize clinical outcomes) trial, patients with symptomatic severe aortic stenosis and who underwent successful TAVI were stratified by gender. The primary outcome was major adverse cardiac and cerebrovascular events (MACCEs), a composite of all-cause mortality or thromboembolic events (including any stroke, myocardial infarction, symptomatic valve thrombosis, systemic embolism, deep-vein thrombosis, or pulmonary embolism). Major bleeding was defined as a composite of major, life-threatening, or disabling Valve Academic Research Consortium-2 bleeding. Of 1,644 patients, 813 were female, and 831 were male. At baseline, female patients were older and at higher surgical risk (Society of Thoracic Surgeons risk score 4.7 ± 3.6 versus 3.6 ± 3.0, p <0.0001) than male patients. After adjustment for differences in baseline clinical and procedural parameters, female patients had lower rates of MACCE (hazard ratio [HR] 0.69, 95% confidence interval [CI] 0.49 to 0.96), all-cause mortality (HR 0.54, 95% CI 0.34 to 0.87), and noncardiovascular mortality (HR 0.33, 95% CI 0.15 to 0.75) at a median of 17 months of follow-up. By landmark analyses, these differences appeared to emerge with a longer follow-up time. No significant differences in major, life-threatening, or disabling bleeding, cardiovascular mortality, and stroke were noted. We contrast how stem cell niches regulate intrinsic properties between progenitor and stem cells throughout development to adulthood. We describe new applications for generating emerging niches from human pluripotent stem cells (hPSCs) using developmental principles and highlight potential applications for regeneration and therapeutics.Distinct tissue architectures arise when cells are organized in specific orientations relative to neighboring cells. These orientational intercellular relationships are established and maintained by cell adhesions. We propose the concept of 'orientational cell adhesions' (OCAs) to couple cell orientations with cell adhesions, thus offering a new perspective to study tissue morphogenesis.

Cylindrical composite resin specimens were subjected to bulk compressive creep test with conventional and modified methodology (with/without introduction of stainless steel hemisphere and preload process). Furthermore, specimens undertook different loading rates ranging from 1N/s to 50N/s. Maximum deformation, creep deformation, permanent set as well as percentage of recovery during the creep and recovery procedure were compared, and surface topography changes before and after preload process was evaluated by laser scanning confocal. There is limited epidemiologic data on sigmoid volvulus (SV) from non-endemic regions. Therefore, we performed a multicenter study to report contemporary outcomes and appraise literature-based methods that pair diagnostic and procedural codes to identify SV.

Using an automated search for patients with 'volvulus' in our system from 2011 to 2021, we reviewed electronic charts to clarify the diagnosis, automatically replicate three strategies to identify SV, and retrieved 6-month outcomes.

Of 895 patients, 109 had SV. Literature-based strategies poorly identified SV. At the index admission, patients underwent endoscopic reduction alone (33%), emergent (16.5%), semi-elective (34%), or elective (16.5%) surgery. Endoscopic reduction alone had high recurrence rates and delayed surgery was associated with worse outcomes.

30 mentor-medical student pairs from the AWS 2020 virtual mentorship pilot-program were surveyed pre-and post-program to explore 1) feasibility of meetings, and, 2) program's perceived efficacy in fostering professional development skills. Participants responded using a 5-point Likert scale (1=not at all; 5=completely). Proportions of participants in each category were compared.

Proportion of participants perceiving monthly meetings to be completely feasible did not differ from pre-to post-program surveys for mentees (75%(21/28) vs. 71%(12/17); p=0.743) or mentors (71%(17/24) vs 71%(13/18); p=1.00). Compared to pre-program responses, mentees endorsed "completely" (Likert scale 5) improving with regard to their elevator speech (p=0.001), developing their curriculum vitae (p=0.003), ability to network (p=0.021), and acquiring skills for career advancement (p=0.003).

Radioactive iodine (RAI) treatment is considered a rare cause of primary hyperparathyroidism (pHPT).

A multi-institutional retrospective review of patients with pHPT who underwent parathyroidectomy from 1990 to 2020 was completed to evaluate the prevalence and latency time for development of RAI-associated pHPT and determine clinical differences in pHPT patients with or without prior RAI treatment.

The appropriate stent length in frozen elephant trunk replacements (FET) remains debated relative to the risk for paraplegia. However, landing the distal end of the stent beyond the curve of the arch facilitates distal reintervention, which is commonly beyond the 10cm stent coverage when deployed proximal to the left subclavian artery. The aim of this study was to evaluate outcomes following the use of 15cm stent grafts in zone 2 (z2, distal to the left common carotid).

Using our single institution-maintained database, 103 zone 2 FET performed from 2016 to 2020 were reviewed.

Of the 103 z2, a 15cm stent graft was used in 51 operations. The indications for FET included acute and chronic aortic dissection, arch aneurysms, and pseudoaneurysms. The incidence of SCI was 0%. Seven deaths (13.7%) occurred.

The data demonstrates the incidence of post-operative paraplegia to be 0% with 15cm z2 FET. The understanding of SCI in FET should not only include the stent length but also from where it begins.

The fate of phenolic compounds in oil and food during cooking vary according to the type of cooking. From a nutritional point of view, reviews largely suggest a preference for using extra-virgin olive oil at a low temperature for a short time, except for frying and microwaving, for which there appears to be no significant advantages compared to olive oil. However, due to the poorly pertinent use of terminology, the different protocols adopted in studies aimed at the same objective, the different type and quality of oils used in experiments, and the different quality and quantity of PC present in the used oils and in the studied vegetables, the evidence available is mainly contradictory. This review tries to reanalyse the main experimental reports on the fate, accessibility and bioavailability of phenolic compounds in cooking oils and cooked vegetables, by considering different cooking techniques and types of oil and foods, and distinguishing experimental findings obtained using oil alone from those in combination with vegetables. The re-analysis indicates that incomplete and contradictory observations have been published in the last few years and suggests that further research is necessary to clarify the impact of cooking techniques on the phenolic compounds in oil and vegetables during cooking, especially when considering their nutritional properties.Outbreaks and prevalence of infectious diseases worldwide are some of the major contributors to morbidity and morbidity in humans. Pharmacophageous plants are the best source for searching antibacterial compounds with low toxicity to humans. In this study, we identified, for the first time, antibacterial components and action modes of methanol-phase extract from such one edible herbaceous plant Rumex madaio Makino. The bacteriostatic rate of the extract was 75% against 23 species of common pathogenic bacteria. The extract was further purified using the preparative high-performance liquid chromatography (Prep-HPLC) technique, and five separated componential complexes (CC) were obtained. Among these, the CC 1 significantly increased cell surface hydrophobicity and membrane permeability and decreased membrane fluidity, which damaged cell structure integrity of Gram-positive and -negative pathogens tested. A total of 58 different compounds in the extract were identified using ultra-HPLC and mass spectrometry (UHPLC-MS) techniques. Comparative transcriptomic analyses revealed a number of differentially expressed genes and various changed metabolic pathways mediated by the CC1 action, such as down-regulated carbohydrate transport and/or utilization and energy metabolism in four pathogenic strains tested. The linearity ranges were 0.1-8 ng/mL for TS I and DTS I, 0.2-8 ng/mL for TS IIA, 1-80 ng/mL for CTS, 20-800 ng/mL for PAL and CFA, and 10-4000 ng/mL for PCTA. The developed method was accurate and precise for the compounds. The relative matrix effect was less than 15%, and the analytes were stable for analysis. Studies have highlighted the action of chondroitin sulfate (CS) in different cells; thus, our aim was to analyze its effect on an experimental model of bile duct ligation (BDL). Adult Wistar rats were subjected to BDL and treated with CS for 7, 14, 21, or 28 days intraperitoneally. We performed histomorphometric analyses on Picrosirius-stained liver sections. Cell death was analyzed according to caspase-3 and cathepsin B activity and using a TUNEL assay. Regeneration was evaluated using PCNA immunohistochemistry. BDL led to increased collagen content with corresponding decreased liver parenchyma. CS treatment reduced total collagen and increased parenchyma content after 21 and 28 days. The treatment also promoted changes in the hepatic collagen type III/I ratio. Furthermore, it was observed that CS treatment reduced caspase-3 activity and the percentage of TUNEL-positive cells after 14 days and cathepsin B activity only after 28 days. The compound responsible for this effect was purified and found to be the hibiscus acid. When tested, this compound also inhibited motility and reduced the secretion of both flagellin and type III secretion effectors. In this paper, we provide an in silico virtual screening and molecular dynamics-based analysis to uncover efficient Dengue infection inhibitors. Based on a Google search and literature mining, a large phytochemical library was generated and employed as ligand molecules. In this investigation, the protein target NS2B/NS3 from Dengue was employed, and around 27 compounds were evaluated in a docking study. Phellodendroside (-63 kcal/mole), quercimeritrin (-59.5 kcal/mole), and quercetin-7-O-rutinoside (-54.1 kcal/mole) were chosen based on their binding free energy in MM-GBSA. The tested compounds generated numerous interactions at Lys74, Asn152, and Gln167 residues in the active regions of NS2B/NS3, which is needed for the protein's inhibition. As a result, the stable mode of docked complexes is defined by various descriptors from molecular dynamics simulations, such as RMSD, SASA, Rg, RMSF, and hydrogen bond. The sausages with chia seed in the amounts of 1% and 2% as well as the sample with 1% addition of black cumin seed were characterized by positive sensory features, and their overall quality was rated above 7 c.u. on a 10-point scale, similar to the control sausage. Sausage samples with the addition of cumin seed were characterized by the highest herbal odor and flavor. The addition of Salvia hispanica and Nigella sativa seed significantly affected the amount of volatile compounds in fermented sausages.

To investigate the impact of computed tomography (CT) image acquisition and reconstruction parameters, including slice thickness, pixel size, and dose, on automatic contouring algorithms.

Eleven scans from patients with head-and-neck cancer were reconstructed with varying slice thicknesses and pixel sizes. CT dose was varied by adding noise using low-dose simulation software. The impact of these imaging parameters on two in-house auto-contouring algorithms, one convolutional neural network (CNN)-based and one multiatlas-based system (MACS) was investigated for 183 reconstructed scans. For each algorithm, auto-contours for organs-at-risk were compared with auto-contours from scans with 3mm slice thickness, 0.977mm pixel size, and 100% CT dose using Dice similarity coefficient (DSC), Hausdorff distance (HD), and mean surface distance (MSD).

Increasing the slice thickness from baseline value of 3mm gave a progressive reduction in DSC and an increase in HD and MSD on average for all structures. Despite the importance of cumulative culture for understanding the evolutionary history of our species, how and when cumulative culture evolved is still debated. One of the challenges faced when addressing these questions is how to identify culture-dependent forms that result from cumulative cultural evolution. Here we propose a novel method to identify the most likely cases of culture-dependent forms. The 'Method of Local Restriction' is based on the premise that as culture-dependent forms are repeand especially the three locally unique forms) identified by our method should be tested further for their potential reliance on copying social learning mechanisms (and in turn, for their potential culture-dependence). We report the clinical description of a young adult male presenting with relapsing ganglioneuromas, dysmorphic features, cardiac abnormalities, and multiple lentigines, strongly suspicious for NS. Solid tumor testing identified the recurrent pathogenic c.922G>A (p.Asn308Asp) in PTPN11. Proband and parental blood sampling testing confirmed c.922G>A as a de novo germline alteration. Comprehensive literature review of solid tumors specifically associated to PTPN11, indicates that this is the first documentation of ganglioneuroma and its clinical recurrence after resection in conjunction with a genetically confirmed NS diagnosis. The findings in our patient further extend the list of neuroblastic and neural crest-derived neoplasms associated with this condition.
Despite alarming increases in suicide deaths among preadolescent children, knowledge of the precipitants of suicide risk and the characteristics of children who seek treatment for suicidality is limited. This study's purpose is to describe children (ages 6-12) hospitalized for suicide-related concerns and compare demographic and diagnostic differences between children and adolescent (ages 13-18) patients.

This retrospective study analyzed medical records of 502 children and adolescents ages 6-18 admitted for suicide-related risk to one psychiatric inpatient hospital in southeastern United States between 2015 and 2018.

Patients were predominantly White (63.5%), female (64.5%), and non-Hispanic/Latino (85.1%). We conducted descriptive analyses and a series of logistic regressions comparing children and adolescents with data extracted from discharge summaries, (i.e. primary reasons for admission, environmental stressors, and diagnostic categories). Common environmental stressors included school (63.2%) andracteristics of children dying by suicide. Compared to adolescents, hospitalized children are more likely to be Black, male, and have a neurodevelopmental disorder diagnosis. Proactively identifying and providing strengths-based supports for Black boys and families appear critical for suicide prevention in children.Child or adolescent psychiatric inpatient units are expensive; therefore, a lot is expected of them. Short stay units are best used for diagnostic clarification, the initiation and supervision of complex treatment, and acute containment of risk. Longer stay units provide a recovery-focused programme to address mental health problems that have not responded to intensive treatment in other settings. Both types of unit form part of the wider system of care that supports young people with mental health problems. The PMC features water tolerance and high structural stability under vacuum and various gas adsorption conditions, which are rare among intrinsically porous organic molecules. The eluted N2 and CH4 are obtained with over 99.9 % and 99.8 % purity, respectively, and the separation performance is stable for 30 cycles. Giant garlic is a functional food that contains different kinds of bioactive molecules with beneficial effects on chronic noncommunicable diseases like diabetes and cardiovascular conditions. Considering biosynthesis pathways, abundance, and biological activity, alliin and S-allyl-cysteine were used as chemical markers of organosulphur compounds present in giant garlic.

To establish a chemometric optimisation of pressurised liquid extraction for the determination of alliin and S-allyl-cysteine in giant garlic by liquid chromatography tandem mass spectrometry (LC-MS/MS).

Samples were blanched (ca. 90°C for 10 min) to inactivate alliinase and γ-glutamyl transpeptidase enzymes and then freeze-dried. Chemometric optimisation was performed via response surface methodology based on central composite design (CCD). Organosulphur compound yields were determined applying a validated LC-MS/MS method in multiple reaction monitoring (MRM) mode using the following transitions for alliin m/z 178 → 74 and for S-allyl-compounds from giant garlic. Extraction yields were significantly (p  less then  0.05) higher than those obtained with conventional ultra-turrax extraction.
The Helicobacter pylori (H.pylori) infection-related diseases, peptic ulcer, and gastric cancer are frequently asymptomatic until the onset of complications. This study aimed to investigate the prevalence of H.pylori, erosive esophagitis, peptic ulcer, and precancerous lesions such as atrophic gastritis, intestinal metaplasia, gastric dysplasia, and upper gastrointestinal (GI) malignancy in asymptomatic Chinese.

From January to December 2017, a questionnaire was administered to consecutive asymptomatic patients undergoing routine physical examination, which included their first screening esophagogastroduodenoscopy. H.pylori infection was determined by one of positive
C urea breath tests or rapid urease test and histology. The presence of H.pylori infection, erosive esophagitis, peptic ulcer, precancerous gastric histology, and upper GI malignancy was analyzed in relation to demographic factors.

An optical coupling method with high alignment tolerance by self-written waveguide (SWW) formation is a promising candidate for co-packaged optics (CPO) by silicon photonics (SiPh). However, conventional SWWs cannot be used with Si waveguides because visible light for SWW formation cannot radiate from the waveguide facet. Here, we devised a new, to the best of our knowledge, optical circuit with SiOxNy waveguides for SWW formation from an SiPh chip. With our circuit, we achieved optical coupling between an SiPh chip and a standard single-mode fiber (SSMF) with a tapered SWW (TSWW). The lowest excess coupling loss compared to butt coupling with a high-numerical aperture (NA) fiber is approximately 0.6 dB over the C-band with the TSWW. In addition, our coupling method has higher alignment tolerances than butt coupling with a high-NA fiber (HNF).We show here that light polarization of a beam propagating through a heliconical cholesteric cell can be controlled by tuning the Bragg resonance of the structure. We demonstrate that this control is achieved by varying either the low-frequency electric field or the intensity of a pump beam impinging on the sample. The study confirms the recently reported phenomenon of optical tuning of the heliconical cholesterics and opens the door for the development of simple and efficient polarization modulators controlled electrically or optically.Spatial division multiplexing (SDM) is one of the most important technologies that may help to solve the future capacity crisis. However, to date, SDM optical amplification is still a challenge for its application. Herein, we numerically and experimentally demonstrated a few-mode Er/Yb co-doped fiber amplifier (FM-EYDFA) for extended L-band operation. A double cladding Er/Yb co-doped fiber was fabricated to expand the L-band bandwidth and a novel, to the best of our knowledge, cladding-pumped pseudo-two-stage amplification configuration was proposed to enhance the L-band gain. With an initial signal power of -16.8 dBm and an injected pump power of 8.8 W at 940 nm, the 20-dB gain range was covered to 1620 nm for two-mode groups of LP01 and LP11. Importantly, the average gain of 25 dB and average differential modal gain (DMG) of less then 1 dB were obtained in the wavelength range of 1570-1620 nm for all modes. Our results suggest that the cladding-pumped pseudo-two-stage amplifier based on Er/Yb co-doped fiber providing low DMG, and broad bandwidth has a great potential for increasing the future SDM capacity.Focal plane array (FPA) detectors have escalated Fourier transform infrared (FTIR) microspectroscopy to a potent hyperspectral imaging method. Yet, despite the instrumental multiplex/multichannel advantages, the fidelity of the hyperspectral images relies on the throughput as the total flux of the source is divided among each FPA pixel. Additionally, maintaining the spectral resolution requires a certain level of collimation of the beam which adversely affect the flux of high étendue source. To this end, we propose an implementation of two deformable mirror (DM) sensorless adaptive optics system for infrared (IR) source coupling. The deflection shape of each DM membrane is optimized individually to deal with the beam intensity and the rays' direction in a separate manner, while preserving the spectral quality across the entire mid-IR range. This paper contemplates the choice of metrics in sequential optimization in conjunction with two variations of stochastic parallel gradient descent optimization algorithm. We discuss this approach with respect to a state-of-the-art FTIR microscope.Realizing a densely packed waveguide antenna array is of great importance in light detection and ranging (LIDAR), owing to its suppressed grating lobes. In this work, a low-cross-talk half-wavelength pitch silicon waveguide array is proposed and experimentally demonstrated. It has a periodic arrangement of silicon strip nanophotonic waveguides, between which deep-subwavelength silicon strips are placed. Our experimental results show that this array's cross talk suppression is nearly 20 dB and has a bandwidth covering a wavelength range from 1500 nm to 1560 nm. Our realization of a half-wavelength pitch waveguide array may offer a promising platform for studying integrated optical phased arrays for solid-state LIDAR with a very low grating lobe and thus potentially a large field of view.In non-Hermitian quasicrystals, mobility edges (ME) separating localized and extended states in the complex energy plane can arise as a result of non-Hermitian terms in the Hamiltonian. Such ME are of topological nature, i.e., the energies of localized and extended states exhibit distinct topological structures in the complex energy plane. However, depending on the origin of non-Hermiticity, i.e., asymmetry of hopping amplitudes or complexification of the incommensurate potential phase, different winding numbers are introduced, corresponding to different transport features in the bulk of the lattice while ballistic transport is allowed in the former case, pseudo-dynamical localization is observed in the latter case. The results are illustrated by considering non-Hermitian photonic quantum walks in synthetic mesh lattices.We systematically demonstrated the angular and temperature acceptances of noncritical phase-matching (NCPM) fourth- and fifth-harmonic generation (FHG and FiHG) of a 1077 nm laser in NH4H2PO4 (ADP), KH2PO4 (KDP), and KD2PO4 (DKDP) crystals. In this work, a new, to the best of our knowledge, laser frequency with a wavelength of 1077 nm was generated by optical parametric amplification, in which the pump light (526.3 nm) was generated by the frequency doubling of a NdYLF laser (1052.7 nm), and the signal light was a YbYAG laser (1029.5 nm). Subsequently, the 1077 nm laser was used as the fundamental wave for FHG and FiHG to obtain a deep-ultraviolet laser source. For ADP and DKDP crystals, NCPM FHG of a 1077 nm laser was realized at 74.0∘C and 132.5∘C, respectively, and large angular acceptances of 59.8 and 61.6 mrad were measured. For the FiHG, NCPM was realized in a KDP crystal at 48.5∘C with an angular acceptance of 56.4 mrad. The results pave the way for high-energy and high-power deep-ultraviolet laser generation using KDP-family crystals under noncryogenic conditions.A binary-lens-embedded photonic crystal (B-LEPC) was designed for operation at 1550 nm and fabricated by multiphoton lithography. The lens is binary in the sense that optical path difference is generated using unit cells having just two distinct fill factors. The unit cells have a "rod-in-wall" structure that exhibits three-dimensional self-collimation. Simulations show that self-collimation forces light to move through the device without diffracting or focusing, even as the wavefront is reshaped by the lensing region. Upon exiting the device, the curved wavefront causes the light to focus. The thickness of a B-LEPC was reduced threefold by wrapping phase in the style of a Fresnel lens. Embedding a faster-varying phase profile enables tighter focusing, and numerical aperture NA = 0.59 was demonstrated experimentally.Continuous-variable quantum key distribution (CV-QKD) is a protocol that uses quantum mechanics to ensure that the distribution of an encryption key is secure even in the presence of eavesdroppers. The wide application of CV-QKD requires low cost, system simplicity, and system stability. However, owing to the particularity of Gaussian modulation in CV-QKD, an amplitude modulator (AM) and a bias controller are required, making the system structure complex and unstable. In this Letter, we achieve two-dimensional Gaussian modulation with only one phase modulator (PM) and a Sagnac ring structure, which significantly reduces the complexity of the system. We test the Gaussian modulation stability for 10 h, and the result shows that the expected secure key rate can be maintained at 80 kbit/s under a transmission distance of 50 km. This scheme opens up new, to the best of our knowledge, possibilities for a new generation of highly stable and simple CV-QKD systems.

Typically clinical decision making about what to do next for a patient incorporates clinical judgement about the diagnosis with severity of symptoms and patient factors, such as their ideas and expectations for treatment. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is associated with late complications that can impair the quality of life (QoL) of patients for years after transplant. The purpose of the present study was to determine the difference in the QoL of adults that underwent allo-HSCT in childhood and adolescence compared with not transplanted adults.

In this prospective case-control cross-sectional study, we included patients aged ≥18 years that received an allo-HSCT during childhood or adolescence and subsequently survived at least 2 years after transplantation. The control group consisted of blood donors matched for age and sex. QoL assessment was performed using the Short Form-36 (SF-36) Health Survey, Portuguese version 2.

Thirty-four transplanted patients and controls were included. 58.8% were male, and the median age at transplant was 13.5 years (range, 4-17 years). The median follow-up was 11.5 years (range, 2.0-23.0 years). The most common late effect was skeletally followed by endocrine complications.
