Meropenem is commonly used in the ICU to treat gram-negative infections. Due to various pathophysiological changes, critically ill patients are at higher risk of having subtherapeutic concentrations and hence have a higher risk of treatment failure-especially in regions where gram-negative drug resistance is increasing, such as Saudi Arabia. No studies have evaluated the pharmacokinetics of meropenem in critically ill patients in Saudi Arabia. Our primary objective is to assess the percentage of patients achieving the therapeutic target for meropenem.
This prospective observational study was conducted in the ICUs of King Khalid University Hospital. Patient were included if >18years-of-age and received meropenem for a clinically suspected or proven bacterial infection. link2 The primary outcome was to assess the percentage of patients who achieved the pharmacokinetic/pharmacodynamic (PKPD) therapeutic target of a free trough concentration four times the MIC. The secondary outcome was to estimate the pharmacokinetics of meropenem. Pharmacokinetic analysis was performed using Monolix Suite 2020R1 (Lixoft, France).
Trough concentrations were highly variable and ranged from <0.5µg/mL to 39µg/mL, with a mean±SD trough concentration of 8.5±8µg/mL. Only 46% of patients achieved the therapeutic target. The only significant predictor of failing to achieve the PKPD target was augmented renal clearance.
In conclusion, more than half of our patients did not achieve the PKPD target. Thus, there is a need for better dosing strategies of meropenem in critically ill patients in Saudi Arabia such as extended and continuous infusion.

Every year, unintentional poisoning contributes to 106,683 deaths globally. In Kuwait, paediatric poisoning cases comprise approximately 50% of total poisoning cases. Despite the extensive importance and the long history of poison control centres (PCCs) and the emphasis of the World Health Organization (WHO) to establish a PCC in Kuwait, no functional PCC exists in Kuwait. Here we reported 82 poisoning cases between July and December 2020, revealing a 100% increase in comparison to the official figures published in 2004 and 2005. No fatalities were reported, and all cases were discharged home within 12 h of their visit to the casualty. Children aged 2 to less then 4 years comprised the most reported poisoning cases with approximately 45% of the total. The number of male child poisoning cases was approximately two-fold of female children. The most common poisoning agent was silica gel granules (9%) followed by medicines - reported as paracetamol (7%), diclofenac (7%), multivitamin gummies (7%) and vitamin C (5%). Among other causes of poisoning were ingestion of salbutamol nebulizer solution (4%), oral contraceptives and insecticides (4%). These findings reveal the importance of establishing a functional PCC in Kuwait to minimise the unnecessary visits following ingestion of expired orange juice and henna, that may encounter further contraction of infections, especially with the current state of the COVID-19 pandemic. Moreover, a functional PCC would provide comprehensive data and hence further intervention such as shifting the dosage form of salbutamol from nebulizer solution to metered dose inhaler with a spacer, in addition to increasing public awareness towards minimizing such a dramatic increase in casualty visits because of -suspected poisoning.Hawthorn (Crataegus spp.) has been used for the treatment of several heart diseases and hypertension. The studies carried out on several hawthorn species have led to the development of standardized extracts useful in the cure of mild chronic cardiac diseases. In Mexico, the most common Crataegus species are C. mexicana and C. gracilior. Decoctions prepared from the fruits and leaves of these species have been employed to the treat respiratory diseases, tachycardia and to improve coronary blood flow. link3 Considering that to date there are no reports of the use of Mexican Crataegus species to treat cardiovascular diseases, we propose an analytical method to obtain a quantified extract of Crataegus mexicana leaves for the development of a standardized extract with therapeutic value in cardiovascular diseases as an alternative source to the extracts obtained from Crataegus species of European and Asian origin. Therefore, the aim of this study was to obtain an extract prepared from C. mexicana leaves with the highest vasodilator activity to select the optimal chemical marker to stablish and validate a reversed-phase high-performance liquid chromatography (RPHPLC-DAD) analytical method for obtaining a quantified extract with vasodilator effect. The results obtained from the analytical method validation, which was carried out according to the guidelines stablished in the Eurachem Guide and the ICH guidelines proved that the RPHPLC-DAD method we developed was specific, precise, accurate, and showed good linearity over the concentration range of 3 - 21 µg/ml for (-)-epicatechin and rutin, which were selected as chemical markers.
Given the widespread use of clinical reasoning (CR) in the healthcare practice, it is essential to inculcate the CR practice in undergraduate pharmacy education which can not only facilitate their clinical education and clinical rotations but can also help them become better clinical pharmacists. There is very limited CR employed in the pharmacy curriculum and practice in the Middle East countries. This study aimed to develop and evaluate CR practice in pharmacy undergraduates in one college of pharmacy in Saudi Arabia.
We employed a mixed-methods methodology that included two phases. In Phase I, students were introduced to CR practice ('think aloud' method) and given geriatric clinical cases which they used in two sessions together with a tutor. This was followed by the writing of SOAP notes using the tutor feedback and completion of a survey that included a self-reflection about their experience of using the CR method. Phase II included face-to-face semi-structured interviews involving selected students method can be widely employed in pharmacy education and practice.This study aimed to develop propolis and tea tree oil nanoemulsion loaded with clindamycin hydrochloride to heal wound effectively. Nanoemulsion formulae were prepared and characterized by droplet size analysis, zeta potential, viscosity, ex-vivo permeation, and skin deposition. The optimal formula was evaluated in terms of morphology, cytotoxicity, and in-vitro wound healing assay. Also, the efficacy of the optimal formula was evaluated by in-vivo wound healing and histopathological studies. The optimal formula (F3) was composed of 9% tea tree oil and 0.4% propolis extracts with mean droplet size 19.42 ± 1.7 nm, zeta potential value -24.5 ± 0.2 mV, and viscosity 69.4 ± 1.8 mP. Furthermore, the optimal formula showed the highest skin deposition value 550.00 ± 4.9 µg/cm2 compared to other formulae. The TEM micrograph of the optimal formula showed that the nanoemulsion droplet has an almost spherical shape. Also, the optimal formula did not show noticeable toxicity to the human skin fibroblast cells. The in-vitro and in-vivo wound healing assay showed unexpected results that the un-loaded drug nanoemulsion formula had a comparable wound healing efficacy to the drug-loaded nanoemulsion formula. These results were confirmed with histopathological studies. Our results showed that the propolis and tea tree oil nanoemulsion, whether loaded or unloaded with an antibiotic, is an efficient local therapy for wound healing.
Self-medication is a common practice and cause for concern globally. There is a paucity of information regarding students' self‑medication in Zambia. Therefore, this study aimed at determining the self‑medication practices among the medical students at Copperbelt University.
A descriptive cross-sectional study was conducted among 334 students. Data was collected using a semi-structured questionnaire, entered in excel, cleaned, and exported to SPSS version 21 for statistical analysis. Univariate analysis using Chi-Square or Fishers Exact test was performed. Independent predictors of self-medication practices were determined using logistic regression. Adjusted Odds ratios (AOR) and their 95% confidence intervals are reported.
A total of 334 medical students, with slightly more females (50.3%, n=168), participated in the study. Of these 61.1% (n=204) reported self-medicating. Reasons for self-medicating were the presence of long queues at health facilities and lack of time to visit the hospital. Only the year of study was independently associated with self-medicating with those in their 4th year of study being more likely to self-medicate [AOR3.43, 95% CI 1.52-7.73].

Sculpture of epidermis varies from smooth to verrucose and rugose in the genus. The abaxial epidermis of some species bears papillae, whereas the adaxial surface uniformly lacks the papillae. Sculpture type of epidermis and density of papillae are generally found to be stable characters at a species level. The infraspecific variation of epidermis sculpture, where present, ranges from smooth to verrucose or from verrucose to rugose. Micromorphological characters of leaf epidermis are shown to have potential taxonomic significance in Aspidistra; in combination with the type of shoot structure, they allow to subdivide the species into 13 groups. The groups are largely incongruent with floral morphological traits. An identification key to the studied species of Aspidistra based on vegetative characters (gross leaf and shoot morphology and characters of leaf epidermis) is presented.
