To (1) describe the distribution of Ministry of Health (MOH) COVID-19 emergency funding to general practices in March and April 2020 and (2) consider whether further funding to general practices should be allocated differently to support equity for patients.

Emergency funding allocation criteria and funding amounts by general practice were obtained from the MOH. Practices were stratified according to their proportion of high-needs enrolled patients (Māori, Pacific or living in an area with the highest quintile of socioeconomic deprivation). Funding per practice was calculated for separate and total payments according to practice stratum of high-needs enrolled patients.

The median combined March and April funding for general practices with 80% high-needs patients was 28% higher per practice ($36,674 vs $28,686) and 48% higher per patient ($10.50 vs $7.11) compared with the funding received by general practices with fewer than 20% high-needs patients. Although the March allocation did increase funding for high-needs patients, the April allocation did not.

Emergency support funding for general practices was organised by the MOH at short notice and in exceptional circumstances. In the future, the MOH should apply pro-equity resource allocation in all emergencies, as with other circumstances.

The primary care response to the coronavirus disease 2019 (COVID-19) pandemic in early 2020 required significant changes to the delivery of healthcare by general practices. This study explores the experiences of New Zealand general practice teams in their use of telehealth during the early stages of the COVID-19 pandemic in New Zealand.

We qualitatively analysed a subtheme on telehealth of the General Practice Pandemic Experience New Zealand (GPPENZ) study, where general practice team members across the country were invited to participate in five surveys between 8 May 2020 to 27 August 2020.

164 participants enrolled in the study during survey one, with 78 (48%) completing all surveys. Five telehealth themes were identified benefits, limitations, paying for consults, changes over time and plans for future use. Heart failure with reduced ejection fraction (HFrEF) is associated with poor outcomes. While several medications are beneficial, achieving optimal guideline-directed medical therapy (GDMT) is challenging. COVID-19 created a need to explore new ways to deliver care.

Fifty consecutive patients were taught to identify fluid congestion and monitor their vital signs using BP monitors and electronic scales with NP-led telephone support. Quantitative data were collected and a patient experience interview was performed.

The majority (76%) of the cohort (male, 76%; Māori/Pacific, 58%) had a new diagnosis of HFrEF, with 90% having severe left ventricular (LV) dysfunction. There were 216 contacts (129 (60%) by telephone), which eliminated travelling, (time saved, 2.12 hours per patient), petrol costs ($58.17 per patient), traffic pollution (607 Kg of CO2) and time off work. Most (75%) received contact within two weeks and 75% were optimally titrated within two months. Improvements in systolic BP (SBP) (124mmHg to 116mmHg), pulse (78 bpm to 70 bpm) and N-terminal pro-brain natriuretic peptide (NT-proBNP) (292 to 65) were identified. Of the 43 patients who had a follow-up transthoracic echocardiogram (TTE), 33 (77%) showed important improvement in left ventricular ejection fraction (LVEF).

Quick COVID-19 Surveys are an international collaboration designed to rapidly analyse and disseminate a primary care perspective on the pandemic and associated health response. In this paper we present results from surveys relating to opening the New Zealand border.

Three surveys were distributed to primary care practices between May and December 2020. A range of primary care member organisations distributed the survey augmented by snowballing. Quantitative data were analysed using descriptive statistics and qualitative data through an inductive process and grouped into themes.

Respondents became increasingly supportive of opening a trans-Tasman border but not internationally. Two broad themes were evident (1) making sure that the border is not an Achilles heel and (2) effective strategies to reduce local transmission. These themes highlight primary care's concerns around management of the border and the management of local spread respectively.

The results highlight concerns around border control from a primary care perspective. The border control issues raised by primary care have proven to be prophetic at times and reflect the role that primary care has as observers of society. The survey mechanism provides a template for rapidly eliciting a primary care voice for future health issues.

Emily spreads revival!

Summary of this testimony: Emily, a CLU student, was able to effectively answer the questions of a prospective student, causing the prospective student to sign up for the Dreams course. They will now grow spiritually over the next several months! As each of us share the good news of the wonderful working power of the Holy Spirit in our lives, others step forward to experience the same. Revival grows!

The question: I’m interested in taking the Dream Interpretation course but I’m a bit apprehensive. I do know Emily you have taken the course. I have watched Charity’s online videos, and my friend let me read her book on it…so I do understand the basics of how to do it. Many times, my dreams are so fragmented. Any help you can offer would be greatly appreciated.

Emily answers: I am so delighted to hear that you are drawn by the Spirit to discover a deeper intimacy with God. Truly the Dream Interpretation Course that CLU offers will not only help you but change your life. It surely did for me. Thank you for asking me about it. I feel honored to be able to share my experience.

Before I explain the details and requirements of the course, I want to address a bigger issue. I would say the only issue you need to be clear about. That is, if you are led by the Spirit to take this course or not. You know how to hear God’s voice from taking the Communion with God Course. You can journal and ask Him. It seems to me that the fear of failure is trying to convince you with reasoning. The fact is that you were interested enough to watch Charity’s streaming video and to read her book about Dream Interpretation, it shows that the Lord is leading your way already.

After I watched the free stream, I thought I had some basic knowledge about the course. But after I finished the course, I realized how little I knew before. When I started the course with expectations, I began having more vivid dreams than ever. And I could remember most of the dreams. I believe that the anointing was given to learn and grow through the course.

God who nudges you to take the course will be with you throughout to reveal what He wants you to know. Truly, I would say that for me everything was done in the power of the Holy Spirit. I didn’t have to strive at all. When you try to strive on your own might, you will not remember your dreams, nor will the interpretation flow from the heart of God. It is better to make it all about Him, His strength, His power, His love, and His desire to communicate with us.

You learned to hear His voice during the day through journaling. When you learn how to hear His heart during the night through dreams, your relationship with Him will grow beautifully.

Don’t worry about remembering only fragments of the dreams now. Cherish what is given. Even just a tiny part of the dreams or a word that you remember, if you ask God from what you have, He will reveal to you what He desires to reveal. After all, it is His desire to communicate with you. The One who brought you this far will lead you through kindly so that He can have intimacy with you.

The reason why you don’t need to worry about whether you can meet the course requirement or not is because I know that when you step out and act on it in faith - faith in action, He provides. I have no doubt that He will reveal Himself to you through your dreams and that according to your faith, desire, and passion, He will let you remember dreams more vividly and fully, and He will give you the interpretations.

As for me, it was more than what I could submit to meet the requirements. The course requirements were just a small part of the whole dynamic of the eternal relationship with God that I cultivated with Him during the time I was taking the course. I have full confidence in the Lord that you will be brought closer to Him, that you will discover how faithful He is, and that you will feel how privileged you are to know the hidden things only the ones who seek can find.

Regarding the course lessons, you will also learn about every dream and vision that the Bible mentions. By doing this Biblical Research, all those people in the Bible who received the intel, clues, and wisdom from above became closer to me. They are just like me. And God communicated to them through dreams and visions. It made me feel that what God is trying to communicate with me every night has such a history-changing power and wisdom. I feel “chosen” to be trusted with His heart of love that communicates through dreams and visions.

It’s all about a journey with Him. You are not taking this course for mere interpretation purposes. It’s more than that. The reason why you want to know how to interpret dreams is like mine, then, you will encounter His faithful love for you through the journey.

The course requires at least 2 dream interpretation samples to be included in each of the twelve lessons. Sometimes I sent out several small fragment dreams and what the Lord says from each of those.

If you are prophetic or want to be more prophetic, I strongly believe that this Dream Interpretation course will help you greatly. You will have a habit of asking God’s heart during the day, as you keep asking Him what certain symbols mean throughout the day.

I'll tell you. I didn’t want this course to end. I wanted it to continue. What an awesome experience I had with God, and the investment of time, money, heart, and passion are all bearing good fruits and it is multiplying. I cannot recommend this course enough. It is a life-changing course.

And remember, Acts 2:17 says, “In the last days,’ God says, ‘I will pour out my Spirit upon all people. Your sons and daughters will prophesy. Your young men will see visions, and your old men will dream dreams”. It’s good to be prepared and be trained so that when the critical time comes in life, you are ready to interpret divine intel and wisdom from above. The beautiful thing is He is already expressing His heart to you every night.

May God bless you abundantly, and be comforted, counseled, and instructed in your journey with Him, as you commune with Him through interpreting dreams and visions.

Comprehensive genomic profiling (CGP) uses next-generation sequencing to analyze the complete coding sequence of hundreds of genes from a small amount of tissue. Genes included in these assays are those associated with cancer development or have diagnostic, prognostic, familial, or therapeutic implications Genomic profiling is emerging as a clinically viable tool to personalize patient's treatment. This article discusses how the insights gained through CGP can impact treatment plan in common gynecological cancers.Head and neck cancers (HNC) are extremely aggressive, highly recurrent, and the sixth most common cancer worldwide. Neuropeptide substance P, along with its primary receptor, neurokinin-1 (NK-1R), is overexpressed in HNC and is a central player in inflammation and growth and metastasis of several cancers. However, the precise SP-mediated signaling that promotes HNC progression remains ill defined. However, its expression and underlying mechanisms are little known in gastric cancer in Indian context. In this study, we detected mRNA expression of VEGF, E-cadherin, and MMPs (MMP-1, MMP-2, and MMP-9) in 73 gastric cancer tissues and 27 normal controls by reverse-transcriptase polymerase chain reaction (RT-PCR). Receiver operator characteristics analysis was done for determining the diagnostic utility of VEGF, MMPs and E-cadherin with respect to the sensitivity and specificity. The association of VEGF, MMPs, and E-cadherin expression with the clinicopathological characteristics and the prognosis was subsequently analyzed. The mRNA expression results showed that E-cadherin was significantly downregulated in 47.9% of GC in comparison to control. There was no change in VEGF expression observed in 90.4% GC cases. MMP-1, MMP-2, and MMP-9 were overexpressed in 13.7%, 28.8%, and 11% of GC, respectively, with significant change in MMP-2 (p ≤ 0.0001) and MMP-9 (p = 0.027) in comparison to control. To date, there are no molecular markers which can reliably predict RCC outcome. We investigated whether a novel kidney cancer marker, carbonic anhydrase IX (CAIX), is associated with progression and survival. A retrospective study was done on patients diagnosed with renal cell carcinoma over a period of 5 years. Immunohistochemical analysis using a CAIX monoclonal antibody was performed on paraffin-embedded blocks from patients treated with nephrectomy for clear cell RCC. Patients were segregated into two categories based on CA IX expression as CA IX ≤ 85% and CA IX > 85%. A comparison was made based on the survival (from date of diagnosis) with CA IX expression. Correlation of CA IX expression and TNM staging, nuclear grading, tumour volume and age was statistically studied using Student's t test. The association between survival and CA IX was done using Mann-Whitney test. The association of CA IX with rest of the prognostic variables were analysed using Fisher's exact test. In our study, CA IX expression > 85% had longer survival compared with those with lower expression ≤ 85%. A significant statistical association was seen with CAIX and lymphovascular emboli, major vessel, perinephric fat, renal sinus fat involvement and distant metastasis. CAIX reflects significant changes in tumour biology that predicts clinical outcome and identify high-risk patients for adjuvant immunotherapy and CAIX targeted therapies.Urothelial carcinoma has a varied and wide histological spectrum posing a diagnostic challenge in H&E examination alone. Immunohistochemical markers like GATA-3 along with other appropriate panel of IHC can be used. However, the percentage positivity and its intensity may vary in different variants and grades of primary and metastatic urothelial carcinoma. To observe the GATA-3 expression patterns in all the grades and different variants of primary and metastatic urothelial carcinomas. It is a prospective and retrospective observational study. All the clinically suspected urothelial carcinoma (UC) during January 2016 to December 2017 were included in the study. Depending on the differential diagnosis considered, immunohistochemistry (IHC) markers including CK7, CK20, p63, AMACR, CDX2, and p16 were done to differentiate UC from other primary carcinomas. The tumors confirmed as UC were analyzed further for GATA-3 expression by Chi-square test. The number of UC in the present study was 126 including 122 (bladder in 107, ureter in 7, renal pelvis in 5, and urethra in 3) primary and 4 metastatic UC (3 in lung and 1 in liver).

Receiver operator characteristics analysis was done for determining the diagnostic utility of VEGF, MMPs and E-cadherin with respect to the sensitivity and specificity. The association of VEGF, MMPs, and E-cadherin expression with the clinicopathological characteristics and the prognosis was subsequently analyzed. The mRNA expression results showed that E-cadherin was significantly downregulated in 47.9% of GC in comparison to control. There was no change in VEGF expression observed in 90.4% GC cases. Patients were segregated into two categories based on CA IX expression as CA IX ≤ 85% and CA IX > 85%. A comparison was made based on the survival (from date of diagnosis) with CA IX expression. Correlation of CA IX expression and TNM staging, nuclear grading, tumour volume and age was statistically studied using Student's t test. The association between survival and CA IX was done using Mann-Whitney test. The association of CA IX with rest of the prognostic variables were analysed using Fisher's exact test. In our study, CA IX expression > 85% had longer survival compared with those with lower expression ≤ 85%. A significant statistical association was seen with CAIX and lymphovascular emboli, major vessel, perinephric fat, renal sinus fat involvement and distant metastasis. Contrariwise, reading labels and believing that upcycled foods are healthier or more environmentally friendly positively impacts on the willingness of Millennial consumers to buy these foods.
